He probably went into cardiac arrest during the procedure and his obesity partly contributed to them not being able to quickly put in the endotracheal tube. But part of it doesn't make sense, if he had a MI (assuming that's what it was) then why did they do cardiac cath on him with him only on monitored anesthesia instead of general anesthesia with intubation? The twitter rumors started from a Japanese tabloid which claimed Kim's botched heart surgery sent him into coma. Those tweets, stories from Taiwan media and funeral photos (doctored or not) are all based on that.
But the real bombshell was not on Twitter, it's on China's Weibo, it was posted by a journalist Qin Feng (currently a news director at a Hong Kong based TV station). Ms. Qin is not a "nobody" reporter, her uncle is Li Zhaoxing, former foreign minister of China. Her social media posts carry a lot weight. She said in her post "...it's fine if you want to wait for the official statement from NK government....but it took 34 hours to announce Kim Il-sung' s death (Kim Jong-un's grandpa), 51 hours to announce Kim Jong-il's death (Kim's dad), how long do you think we will have to wait this time?..."

North Korean dictator Kim Jong Un is in a “vegetative state” after he underwent heart surgery earlier this month, a Japanese magazine says.
The weekly Shukan Gendai reported Friday that a Chinese medic sent to North Korea as part of a team to treat Kim believed a delay in a simple procedure left the leader severely ill, Reuters reported.
North Korean media hasn’t mentioned Kim’s health or whereabouts, even though reports by other media have sparked international speculation about his well-being.
The Chinese expert told the magazine that Kim clutched his chest and fell to the ground on a visit to the countryside earlier this month. A doctor accompanying Kim performed CPR and took him to a nearby hospital.
Kim, believed to be 36, needed a stent procedure, which calls for placing a tube into a congested blood vessel to allow blood to keep flowing to the heart, according to Shukan Gendai.
The hashtag #KimJongUndead was trending on Twitter but without any proof of the leader’s death.
Earlier this week, Seoul-based website Daily NK reported that Kim was recovering after undergoing a cardiovascular procedure on April 12. The outlet cited one unnamed source in North Korea.
South Korean government officials and a Chinese official with the Liaison Department challenged subsequent reports suggesting that Kim was in grave danger after surgery, Reuters said. South Korean officials said they had detected no signs of unusual activity in North Korea.
President Donald Trump has downplayed reports that Kim is ill, and an official familiar with U.S. intelligence said the government had no reason to conclude he was seriously ill or unable eventually to reappear in public, Huffington Post reported.

If that story is true, he’s probably got brain damage either from being denied oxygen or from a stroke. He’s not likely to come back from that. Kim Byeong-ki, a member of the South Korean parliament’s intelligence committee, said on social media Sunday that there was no indication Kim Pyong Il could be the successor.
“I laugh off these theories,” he said.
When Kim Jong Un took power in 2011, he went on a purge, executing his uncle Jang Song Thaek, and was believed to have ordered the assassination of his exiled half-brother, Kim Jong Nam, in Malaysia.
The fact that Kim Pyong Il emerged unscathed may suggest that Kim Jong Un never saw him as a credible adversary, Bloomberg reported.
He is now mostly deemed to be a contender by those who dismiss Kim Yo Jong due to her age and gender, Rachel Minyoung Lee, a former North Korea analyst with the US government, told the outlet.
“It is highly unlikely that he has the connections or the support base he needs to be the next North Korean leader,” she said.
“Kim Yo Jong has a special status in the regime, and I think in this case, her connection to the Kim family trumps her gender.”

R551, women in some Asian cultures aren’t allowed to be bosses, and aren’t taken seriously, and NK is known to be conservative. It makes sense, since the Kim family has total control of NK. It’s to their benefit to have half the population have no power in any way, at work or at home.
About Kim Yo, that’s one of those things where something is impossible until it isn’t, like Obama. But I wouldn’t be surprised if they find some random male that isn’t as qualified as she is and put him in as a figurehead. She could still be important behind the scenes, or she could be executed and got out of the way too.

I think more people think he's alive now than dead. It would make sense that he would hide out away from the Rona, which is probably raging in Pyongyang. Though the fact that they haven't released some footage of him doing something official may mean he is layed up with heart issues.

But he's supposed to be invincible in the eyes of the North Korean citizens. He can't admit he's in hiding. If he could have, he would have shown up for the celebrations. He couldn't show up. If he's not dead, he's dying or in grave condition.
Also, what's with them putting out a letter? They could have shown a video of him sitting in some garden or, like someone suggested above, indicated that he was climbing mountains and skydiving and driving tanks and playing in submarines or, literally, anything but all they could manage is a supposed letter?

No he's not invincible, the citizen know he is mortal.
But he has stated their is no coronavirus, he knows that is not true, so he has to stay in hiding.
He can't have a credible video of him unless it's in a public place, and he won't do that. A video in a private place would be dismissed as being taken earlier. And he isn't going to hold up a newspaper or such with a date, simply because he rules and makes the rules not other people.

Taiwan’s intelligence chief said Thursday that North Korean leader Kim Jong Un is “sick” — and his country has emergency plans for a potential power struggle if he dies, according to reports.
Chiu Kuo-cheng, the director of the National Security Bureau (NSB), gave a vague health description when pressed at a government hearing about rumors of Kim’s possible death, according to Taiwan News.
He merely laughed and refused to reply when asked if that meant the Hermit Kingdom’s leader was indeed alive, the report said.
“My answer is from the intelligence that is available and it is not an opinion,” Chiu told lawmakers, according to the Taipei Times.
He refused to elaborate, citing the need to protect the intelligence service’s sources in North Korea, the report said.
He assured legislators that contingency plans were in place ready for a potential power struggle in the region if the North Korean leader did reply, according to the reports.
However, his deputy, Hu Mu-yuan, later insisted that their intelligence was “unable to confirm the status of Kim Jong Un’s health,” the paper said.
“From what we know, Kim is still in charge and in control of North Korea’s military and its government,” Hu said according to the Taipei Times.
