I don't know about other service branches but the USMC wears ribbons in place of medals on dress uniforms (office attire). It's more comfortable, not as heavy. Usually the riflery medals are the only ones that are actually displayed on the chest, under the ribbons.

And the reason he jumped from 1 star to 3 star is that Mattis wanted to put him in a position that called for a 3 star so the Navy threw 2 more stars on his shoulders. Some jobs in the military can only be filled by certain ranks so that's how Harward went from 1star admiral to 3star admiral in one big jump. Mattis wanted him and the Navy handed him over.
