Monkey Pox spreading in London - clustering among gays and bisexuals

Because it was first identified in 1958 as a pathogen of crab-eating macaque monkeys. It naturally lives in a variety of African animals though, including some rodents. In the U.S. smallpox vaccinations stopped in 1972. I pulled this up from googlie. Looks like monkeypox has been around for awhile.

To protect people from monkeypox, the Jynneos vaccine was approved by the U.S. FDA in 2019. Jynneos (Imvamune or Imvanex) was also approved by the European Medicines Agency in 2013 and is indicated for preventing smallpox and monkeypox disease in adults.

I did read the disease tends to hit children much harder than adults, vaccinated or unvaccinated. Hoping that the vaccines, which always wane over time, also have the T and other memory cells protectiveness at some level that we hear about with COVID, which is why people are getting sick but most people do not become seriously ill and recover at home on their own.

There is some positive news in that the smallpox vaccine (assuming availability) does help if given after exposure. Given the disease transmits slowly an d inefficiently when it manages to spread, case and contact tracing probably will be able to control this outbreak and it will fizzle out.

There are nine confirmed cases in the UK, for example - that makes it relatively easy to contact trace. That is a good thing. (And only six are in gay or bisexual men, so it's not just us, though you can't pretend we're not heavily present, either.) Contact tracing failed in COVID because exponentially it exploded... no one could contact that many people. There is no indication this thing spreads like that thing, so that's something good.

Still, probably good advice to whore it down a notch or two for awhile. I don't think of myself as being sex-negative so much as complexion-positive.

'We're asking the public to contact their healthcare provider if they have a new rash and are concerned about monkeypox.'

The CDC also notes that many of the lesions that appear as a result of monkeypox infection may have similarities to symptoms of STIs like syphilis, herpes, HSV, and others. The agency also warns says that even people who are not gay or bisexual men should be on the look out.

The prevalence of cases in the UK, for which nine cases have been confirmed so far, also puts America in particular at an increased risk of of outbreak.

Dr Jennifer McQuiston, a senior official at the CDC, told Stat News that: 'there's a lot of travel between the U.K. and the United States and other global area,' she said.

He warns that it it likely that more cases of the rare virus in the U.S., though it is unclear whether case figures will eclipse the record 43 cases that were detected in America during a 2003 outbreak.

The Centers for Disease Control and Prevention (CDC) is warning that men who have sex with other men seem to be most at risk at the moment, as it is traveling through their sexual network, all healthcare providers should be on alert.

A possible case of monkeypox was being investigated in New York City after a patient with symptoms arrived at Bellevue Hospital, health officials said Thursday afternoon.

The city’s Department of Health and Mental Hygiene said their health lab will conduct a preliminary test for monkeypox. If the results are positive the samples will be sent to the Centers for Disease Control and Prevention for confirmation.

The patient is being cared for with all isolation protocols in place at NYC Health + Hospitals/Bellevue.

Epidemiologists are expected to follow up with any individual that may have been in contact with the patient while infectious, the health department said

The first case in the US this year was identified Tuesday in Massachusetts. The disease has been found in other countries with more than 60 possible cases identified so far.

Monkeypox is rare in the US, the department said. When a positive case is identified, the symptoms include a flu-like illness in addition to swelling of the lymph nodes and followed by a rash on the face and body.

SEX WARNING AS MONKEY POX REACHES AUSTRALIA

One of Australia’s top doctors has issued an urgent sex warning after killer virus monkeypox was found on our shores.

A Victorian man is in hospital after being diagnosed with monkeypox, while another in NSW is also suspected of being infected with the virus.

The Victorian case is linked to one of nine cases that have emerged in the UK, with some of them connected to the country’s gay and bisexual community.

NSW chief health officer Dr Kerry Chant urged gay men to be particularly vigilant.

“We know it’s transmitted by that close skin to skin contact - you can be infectious and that close droplet contact in a very sort of close prolonged way,” Dr Chant said.

“We’re particularly urging men who are gay or bisexual, or men who have sex with men, to be aware of any unusual rashes or lesions and to contact by phone a sexual health clinic or GP without delay if they have any concerns.

“It is important to be particularly vigilant if you returned from overseas from large parties or sex on premises venues overseas.

“You can imagine that some settings, such as sex on premises venues or other events and gatherings may lead to sort of what we’ve seen as super spreading events.

“It is important that people who have recently returned from Europe who attended such parties be particularly alert given the worldwide case reports today.”

He said the man had developed symptoms prior to boarding the flight, then sought medical care almost immediately after arriving home.

“(He) had an extremely astute GP, who has thought of monkeypox and referred him for testing, which has led to early diagnosis, early isolation,” Dr Sutton told reporters.

The man is in a stable condition at The Alfred Hospital and is expected to make a full recovery.

“There are very few close contacts that have been identified. Obviously, the GP is one of them,” Dr Sutton said.

“We’ll be contacting individuals who are close contacts on the flight — so really in a couple of rows in front and behind — and then people who are on the entire flight, we are just asking to be aware of the symptoms that might occur.

“Those symptoms are flu like symptoms — they can be headache, fever, muscle aches and pains.”

Dr Sutton noted there would be people in Australia born after 1980 who would not be vaccinated against smallpox, which would protect them from monkeypox.

He said Australia should expect to see more cases emerge but did not expect border closures.

Meanwhile, NSW Health said a man aged in his 40s was another “probable case”, saying he had recently returned to Sydney from Europe.

He developed a mild illness several days after arriving home, then went to his GP with symptoms clinically compatible with monkeypox.

Urgent testing identified a probable case of monkeypox, with confirmatory testing underway.

He and his household contact are isolating at home.

“(NSW Health) has also been in contact with sexual health services to increase awareness of the cases identified overseas and to provide advice on diagnosis and referral,” she said.

“We will be speaking with GPs about this issue again today.”

Monkeypox is a rare viral infection that does not spread easily between people and is usually linked to travel to Central or West Africa.

“Cases are occasionally reported in non-endemic countries in returning travellers or their close contacts, or in owners of imported pets,” Dr Chant said.

“People can contract monkeypox through very close contact with people who are infected with the virus.

“The infection is usually a mild illness and most people recover within a few weeks.”

Monkeypox has been identified in several non-endemic countries in recent weeks, including several European countries and the United States.

Another summer written off... in 2019, I packaged out of a job... partly because I hated the job, partly because I knew I was very very rapidly reaching my best before date as a gay man. Portugal, which had confirmed 20 cases as of Thursday, today became the first country to publish a draft genome sequence of the virus.

The data, obtained from a male patient whose infection was confirmed on May 4 after he spotted skin lesions, will help scientists determine the origin and international spread of the currently circulating virus.

The outbreak has been described as 'unusual' by experts because person-to-person transmission of monkeypox was thought to be extremely rare.

Before May, the UK had only ever seen seven cases of the virus, which is endemic in West Africa.

It is usually spread through handling infected animals, either through their lesions, blood, bodily fluids or eating poorly cooked meat.

But it was known that it could be passed on between humans through close contact with the likes of body fluids, respiratory droplets and lesions.

This is why experts think the virus is passing through skin-to-skin contact sex, even though this has never been seen until now.

A similar pattern is emerging in Europe. Seven gay or bisexual men tested positive in Spain.

MailOnline this week revealed close contacts of monkeypox cases, including NHS workers, are already being offered the Imvanex smallpox vaccine.

The strategy is known as ring vaccination, involves jabbing and monitoring those around an infected person to form a buffer of immune people to limit the spread of a disease.

A spokesman for the UKHSA did not disclose how many have been vaccinated, but said: 'Those who have required the vaccine have been offered it.'

Professor Geoffrey Smith, from the University of Cambridge, who advises the World Health Organization on virus research, said: 'It is sensible for doctors and nurses who may be treating patients with monkeypox, to be vaccinated against smallpox.

'This outbreak of monkeypox is highly unusual, but it is very likely the precautions being taken will mean it comes to an end quickly.'

Although designed for smallpox, Imvanex can protect against monkeypox because the viruses causing the illnesses are related.

Data shows it prevents around 85 per cent of cases, and has been used 'off-label' in the UK since 2018.

The jab, thought to cost £20 per dose, contains a modified vaccinia virus, which is similar to both smallpox and monkeypox, but does not cause disease in people.

Initial monkeypox symptoms include fever, headache, muscle aches, backache, swollen lymph nodes, chills and exhaustion.

A rash can develop, often beginning on the face, then spreading to other parts of the body including the genitals.

The rash changes and goes through different stages, and can look like chickenpox or syphilis, before finally forming a scab, which later falls off.

Monkeypox has an incubation period of up to 21 days, meaning it can take three weeks after an infection for symptoms to appear.

Tecovirimat's brand name is Tpoxx, which was mentioned above. There's also cidofovir, which has been around much longer than TPoxx but has some nasty side effects. Chimerix also has antiviral, but they seem to be stuck in the approval process.

The vaccine can also be used post exposure but there's a limited window. They're useless once symptoms appear.

And, keep in mind...all of these drugs were tested using animal models in early phase testing like monkeypox, rabbitpox, etc because smallpox doesn't exist outside a lab and orthopoxviri are all very closely related.

About Cidofovir--- It suppresses CMV replication through selective inhibition of viral DNA synthesis. It was manufactured by Gilead and initially approved by the FDA in 1996, but has since been discontinued.

I googled and that's what came up, maybe it's no longer made by Gilead.

Couple key tweets from the chain:

We know there were imported cases in London and that’s where the first non-travel associated cases were identified. I’d expect the epi investigation to find some travel link between the clusters in different countries leading back to London.

If there’s spread within a sexual network followed by travel, that could easily explain why clusters are popping up in other countries within those extended networks.

Monkeypox has a fairly long incubation period. Lots of time for people to travel before the first vesicles pop.

Btw, this is not specific to MSM. Sex in general is conducive to spreading monkeypox—again, probably plenty of heterosexual sex-associated household transmission. Come at me with your stigmatizing homophobic takes & I’ll reward your efforts with an immediate block with prejudice.
