Also, the report of the lid on the water tank was wide open.

The closed lid was a false report.

It's possible she still couldn't get out once she climbed in, whether the lid was open or closed. She perhaps didn't have enough strength or also could have slipped while holding onto the lip of the opening and was unable to scale the wall to get back up and out.
