Season two of Space/ 1999 had many problems, not the least of which was the jettisoning of three great characters: Victor, Kano, and sexy, mustachioed Paul. Sandra and Dr. Matthias left after the first two episodes.
The show did not get a third season, deservedly so.

She reappeared 13 years later for a brief cameo in Star Trek the Motion Picture.

Taxi- John Burns as Randall Carver. The pilot made it seem that this character would be second only to Judd Hirsch. Gone by the end of the first season. Never mentioned again.
Wings- Brian Haley as Budd Bronski. Brought on in season 7 to replace Lowell. Disappeared without explanation after only a few episodes.

R40, there's an episode S2 that was only shown once. Chuck comes out as gay. Has a huge argument with Mr. & Mrs. C. Gives Potsie a passionate kiss, asks him to sit on it. Potsie says no. He settles for Ralph's Malph. Chuck moves to San Francisco. Has a cameo in Milk.

Wait, I thought Chuck was killed in a boating accident with Richie. Richie survived and went through a tremendous amount of guilt over Chuck's death and he was briefly hospitalized. Marion never seemed to forgive Richie and when he confronted her about never visiting him in the hospital and how he would have visited Chuck, Marion responded, "Chuck would never have been in the hospital!" After that episode the series took a decidedly lighter turn...
