I see them at the beach-guys who look like they are in perfect shape, no scars that would indicate surgery or anything, everything seems to be absolutely fine, except for that coat hook on their stomach.

Well, there are innies and outies.

Mine isn’t disturbingly extreme like what OP describes, but it’s an outie. I was born prematurely and with jaundice, and my guess is maybe they rushed a bit quickly to tie the umbilical cord so that they could give me medical care.

I have a later-life outie - probably because I'm a little "over weight". I'm not a MAJOR fattie - but I'm and ElderGay and I do have a belly....

For the record, I don’t mind an outie. But when it is as pronounced, as you rarely see, that is what I’m curious about. And maybe people who have any innies we have one that goes just as deep inside as the rare few go outside.

While most outies are completely normal, some people end up with an umbilical hernia which happens when there's a gap in the muscle directly behind the belly button. A section of the intestine can protrude through that gap, resulting in an outie.

Well, this is interesting.
When you’re born, the umbilical cord is cut and you have a small piece left called the umbilical stump. One to 2 weeks after birth, this stump falls off and what remains is your belly button.
As a result, your belly button is essentially a scar. Whether it’s an innie or outie depends on how your skin grows as it heals. The way your belly button looks is mostly by chance.
