I think the key issue isn't so much mental impairment (of whatever kind) per se, but just the starkly different treatments of Biden's gaffes and Trump's gaffes. "Gaffe" isn't even an adequate term. Biden was basically coherent and capable of heading an effective administration (ad libbing at the Republicans during the SotU etc.), and the mainstream press kept up a constant drumbeat of questions about his mental state based on anonymous sources. Trump's administration is a constant dumpster fire of incompetence and malevolence, and he himself answers every question with "I don't know, ask my lawyers," yet the mainstream press takes every opportunity to rationalize or shift the focus to political strategy.

The press has two completely different standards for Democrats and Republicans, and it's been getting worse for decades.
