Not really "Hollywood" (though she did have a part in Conan the Barbarian), but the German model/actress Nadiuska went fully schizophrenic, was committed, and lives in a psychiatric hospital somewhere in Spain. They even made a TV series about her.

Brilliant actor, frighteningly menacing and violent IRL.
A couple years ago my sis was visiting from TX, and we watched (she, only half-watched) [italic]Born to Kill[/italic] on TCM.
All of a sudden Sis bolts upright, asking "Wow, who is THAT?" It was L. Tierney, in all his evil glory that was at once oddly hot and creepily cold. After the film was over i told her all the shit he pulled offscreen IRL, but middle Sis always had a thing for sexy bad boys. Even after she read his Wiki, she still has the hots for him.
But then I remembered in her teens she had a longtime thing for Bobby Beausoleil and Tex Watson, of the Manson Family.
She was like "See, Claire Trevor didn't know how to handle a man like that--she was a fool to think she could control a stalking jungle cat, and she got played. GOOD!"

I came across a documentary on Marlon Brandon’s last days on YouTube the other day. The way his close friends and family talk about him, yeah he was a crazy awful human being. He tortured the people closest to him emotionally, especially women. At one point his brother insinuated that he had thought Marlon might shoot him if he came to visit Marlon once he was bedridden and on ventilators because he said he knew how his mind worked (Marlon’s) and he wouldn’t want to die alone.

His daughter Pola claimed he sexually abused her, starting at the age of 5. Her half-sister Nastassja expressed support for Pola and claimed that their father tried to do the same with her but didn't succeed. "He was no father," she said. "Ninety-nine percent of the time I was terrified of him. He was so unpredictable that the family lived in constant terror."
