I think his meds were for bipolar I not schizophrenia. It is prescribed for both and helps manage the manic side of bipolar. He went off and now sounds grandiose. Heavy drinking as is often done to mitigate mania. Extreme mania can include psychotic symptoms too.

R51 poorly phrased, I’m sure, but my question is can they come back from the point where they not dealing with reality to a place where they can appreciate their actions - however real they seemed or may still seem to them - as inappropriate, whether with drugs and or therapy, and not just return to fairly normal functioning but also have an appreciation that their behavior was off the charts in terms of appropriateness. Not so much, “take your meds and you won’t do the things like that” but insight as to how that behavior led, here, to ten or twelve days in jail so far and what sounded like a 5150 hearing to determine his competency held on Tuesday.
Short way to ask: can one come back from that level of delusions/hysteria and not only regain normal function well enough, let’s say, to go back to his RE job, but also appreciate what he was doing when he was delusional or hallucinating? Perhaps by viewing the tape @ R13?

Some may recall things they said or did but not feel like they had any control over those things, or rather weren't in the "driver's seat," some only vaguely recall things they said or did and some have no idea at all. Sometimes it's a combination of all three.

Yes, if they're on their meds but if you've known someone before they had this (grew up with them) and after, they're quite a bit different. They can live functional lives but regular therapy helps as well as (which is required) regular psychiatric evaluations.
The bigger problem is that you constantly have to check in with these people to make sure they're okay. So if your parents or siblings are too into their own lives and your medication needs change then you have to trust your therapist or psychiatrist will catch it. If they don't then your friends or family should. It's a real bitch of a problem.

I've come to this thread to throw a little water on the fires of some of these over the top speculations.
Steroids don't cause the sorts of behaviors you see in Conover. They don't even contribute to them. No causal effect of steroids on violent behavior has been found, as much as you hear about it from unvetted sources. What is most likely is that steroid abusers already have a psychological profile that predisposes them to aggressive behavior. The steroids only contribute to their self-image as being big, bad and tough.
One needs to be careful when diagnosing bipolar disorder in stimulant abusers, because the effects of, for instance, meth or cocaine abuse closely mimic the effects of BP - especially crystal meth. This was a big problem in the late 1980s and throughout the 1990s, when mental health professionals weren't familiar with the effects of crystal meth, and misdiagnosed drug abusers as having BP. Drug abuse masks and/or confuses the symptoms of other mental health conditions, leading to further diagnostic difficulty. For instance, chronic meth abuse causes paranoia and disordered thinking, but it's not schizophrenia.
Conover may have a major mental illness, but until he's clean, it won't be easy to tease out whether it's something like BP or maybe schizophrenia, or maybe just too much ice. Like many successful SM types and real estate people, he's overconfident. The drug use amplifies this grandiose behavior.

R65, the psych meds can reduce the effects of stimulants. I'm still going with drug abuse, whether or not experiencing mental illness. This story is super reminiscent of that of the late G. Bruno, who lived a similar influencer lifestyle, abused drugs and maybe was mentally ill, and then (allegedly) killed somebody with his car, before fatally injuring himself in another vehicular misadventure.
I could also go on and on and on and on about my brother-in-law, who is a poster child for this sort of dual diagnosis lifestyle. It's difficult to tease out which behaviors are drug influenced and which are MI influenced. But it's obvious from 2 decades of observation that even though still nuts, he's much more tame when he's not abusing drugs, even during the times he's off his meds. Meth sends his delusions, grandiosity and aggression through the roof.
Of course people are prescribed psych meds for reasons. But I'm also aware through personal and professional observation that misdiagnosing stimulant abusers with bipolar disorder is a real thing and still happens frequently. If I had it my way, anybody about to receive a dx of BP (or schizoaffective disorder) would be drug tested first and repeatedly to rule out SA. That rarely happens. But it did happen in the case of my BIL, who for good reason was given a court ordered 2-month inpatient detox and another year and a half of compulsory psych meds and weekly drug monitoring and therapy. This helped him a great deal, but I admit that this sort of treatment plan is resource intensive and not typical.

Active addicts often talk about their recovery. Preach, even. It means nothing in terms of their actual sobriety or lack thereof. What they don't talk about is their current SA, which they hide. Wise people never take an addict at their word, especially regarding their substance use.
Dual diagnosed people will frequently abuse any and all substances they can get their hands on: Alcohol, pain meds, muscle relaxants, stimulants, MJ, etc. Usually many types at a time.
Van C. may or may not be abusing substances. He needs and deserves help regardless. I'm only explaining how SA and MI are difficult to tease apart, even for professionals; and to provide evidence of how common it is for people with MI to also abuse drugs. I also provided some personal observations how DD people present IRL.
In the case of my BIL, he presented with behavior equally as crazy and violent as Conover. But he only behaved this way when he was high. When he was sober (but still schizoaffective), he was not as combative or threatening. I realize that one could be this aggressive without the use of substances. But it is far less common.

What I'd add is that what he needs is long term psychiatric hospitalization to stabilize, at least six months with a long stay in a halfway house and intensive follow-up. He needs containment if he's to be treated successfully. Well, he's going to get the containment now.
One of the problems of getting people who are so delusional and psychotic into treatment involves commitment laws. It's hard to hospitalize someone unless they're a danger to themselves or others. Some people have to decompensate to that point before intervention occurs. Also, they do have the right to refuse medication unless under conservatorship (Britney).

His attorney needs to broker a NGRI deal-that’s not guilty by readon of insanity. He was apparently having a scozophrenic episode, the should be doeabke. He’ll spend time in a state mental health facility, where he needs to be. This is not an instant get out of jail free card. He can’t take a coupke pills and a few eeeks later get released because he’s been restored to normalcy. It’s not that simple.. He’ll be there for a while, but the important thing is that he will not have been convicted of a felony. Problem is, there are a dozen good looking men with licenses in NYC who even now are competing to take over EVC’s YouTube niche.

My dad is a psychiatrist and I once asked him - kind of playing the devil’s advocate - why schizophrenics didn’t attempt to hide their hallucinations. Why didn’t they perceive they were “different” than others and tamp it down? I think I was referring to a film character at the time but extrapolated it out to real life.
Anyway, the simplest answer is that their reality is the only correct one. They don’t have the capacity or insight to say, “hey, people look at me funny when I’m talking to the purple bunny walking alongside me but that’s their problem because he’s right here so I’m going to keep talking to it.”
In Blithe Spirit, the Rex Harrison character is caught several times by his current wife talking to his dead wife and he eventually learns (IIRC) to hide her existence for a time. That film trope has always frustrated me - you know it looks weird so hide it from others who aren’t yet in on the secret!

But it's horrible to see someone, anyone so out of command of himself. And to realize that not matter how careful and attentive he could have been to looking after his mental health and staying on his meds, he was never very far from real madness. Of course that doesn't begin to excuse running over a man. It doesn't mean he's a nice person under it all. It's not about making apologies only having a bit of compassion for how terrible mental illness can be and how close to the surface.

R122 That's true. I'm hoping after he is stabilized and stays on his meds he'll be ok long term. I think this attempted murder thing is way overblown. The officer was not badly hurt, not even hospitalized. Im hoping charges will be reduced and possibly get off without time, but he has to be on parole a long long long tibe, a condition of which is that he see's a psyc dr. Nurse pract, or pav weekly to ensure he is on his meds. This is the only way he'll escape your scenario. I understand mental illness and i truly feel for him.
