This is absolutely a terrible photograph, and she looks bad. There's too much contrast in her makeup, it's harsh and there's too much of it. If she'd been made up differently she wouldn't look so deathly. She actually has plenty of fat in her face. Her hair looks like straw, and the dress is terrible. A good stylist and a good makeup artist could do her a world of good.

Heroin addicts don't tend to be super new-age-y types. They're too busy nodding off and enjoying the feeling of being tranquilized. Most meth heads, on the other hand, develop meth psychosis after prolonged use, and oftentimes have really whacky spiritual ideas that are elaborate and have their own consistent logic, despite how nuts the ideas often are. They also don't eat consistently while tweaking, and are awake for days before subsequently crashing (which also lasts for days). Meth is also rougher on the body than heroin. She looks like a meth addict 100%.

It's not the desert, but rural areas in general. They're more likely to be left alone to their own devices without interference, so they can hoard all sort of junk on their property without neighbors to complain. Plus it's cheaper to live there so more $$ can be dedicated to the habit

They also like to be closer to the source. A lot of meth cooking happens in sparsely populated rural areas where the awful chemical stench can disperse with minimal notice.

Back in the late 80s and early 90s at least up in Yolo county outside of Sacramento, some took to driving around in vans that contained mobile labs to disperse the smell and make it harder for LE to pinpoint them. It's around the time the mentally ill really started using meth and their symptoms and presenting problems were such exaggerated versions of their previous presentation. One of the precursors to the homelessness problem now. It's their drug of choice.
