He's getting older. He's a middle aged man now. Things change physically.

He looks the same to me. His face changed forever after Dallas Buyers Club when he lost so much weight and all his buccal fat and the fat pads below his eyes and even on his forehead. It sunk his face in a little (not that he looks bad per se). You could tell when did the 1st season of True Detective. He looked very different. It seems to have stuck that way and the fat / fullness of his face was lost.

I think he still looks pretty good.

It's the sun.

Matthew is an outdoorsman. He spends a lot of time surfing and doing tons of outdoor activities.

When you're young, it doesn't show. But when you hit Matt's age, the sun damage becomes evident.

With all the time he has spent in the sun, I'm actually surprised that he hasn't had melanoma.

If that ever happens, he probably won't be able to go out in the sun any more.

Conversely, Matthew's son Levi is lucky.

He has darker skin, so the sun won't impact him as much because of the melanin in his skin.

Unfortunately for Matthew, his pale skin couldn't take all the sun damage.

How the hell did Jared Leto and Christian Bale lose excessive weight and not look as drawn and lumpy like McConaughey?

R16 that's lower lid surgery, a facelift and a chemical peel. At least I hope that's it because it might yet look better in half a year then.
