Once you hit a certain age, you should get a pneumonia vaccine.
You can significantly reduce the chances of a lot of very bad things with vaccines after you hit 50 that they don't really warn you about: Shingles, pneumonia, tetanus (you do need to get a booster for this periodically).

Meh. I was hospitalized twice in one year with Community Acquired Pneumonia. And I'm 25 years younger than Gere. (And I'm twice as handsome and my dick is 7 times as long).
But the second time I was hospitalized, they really freaked me out. First a nurse was doing the intake and she casually says "and you have HIV?" 😳
I said, no, I take PrEP and if you look at my medical labs, you'll see my last HIV test was negative. And she said OK, but then the attending doc who never even examined me added HIV to my medical history and wrote in the notes "Pt denies hx of HIV".
When I was released I had to contact my primary care doc and say "they listed HIV on my medical history, but unless I was tested without consent AND not informed of the results, I'm HIV negative to my knowledge". My doc removed HIV from my medical history, but I had to file an appeal to have the ER doc change his notes that said I had HIV (and was denying it.) I even went in and got tested for my PrEP check up two months early just so I could prove and have a lab report that I was negative.
Weird thing was, I had pneumonia, yeah, I felt pretty crappy, but I wasn't in acute distress. And the ER was really crowded, but instead of waiting my turn, they rushed me back and started an IV the moment I walked in the door. I think they thought I had HIV and was had PCP because they saw the Truvada listed in my current meds list OR (and this is kinda what I think really happened): they saw that I had a prescription for Truvada (2 ARVs) and had been prescribed Paxlovid (which contains another ARV) and they may have had a query that looked at my info, saw that I had been prescribed THREE ARVs and a data error occurred and auto-coded me as HIV+.
The reason I suspected the latter was because it was a hospital I had worked at previously, I had worked in the data center and worked in EPIC, the electronic medical records system, and we were always creating queries to try to catch things before they slipped through the cracks. If Paxlovid hadn't contained ritonavir, *I* very easily could have been the guy that created that query as a safety net to help identify potential HIV cases that weren't documented. That's basically what we did all day and were there for.

Pneumonia can be caused by a wide variety of bacteria, viruses and fungi in the air we breathe.
The pneumonia vaccine helps protect against serious infections caused by 23 types of pneumococcal bacteria, but not all of them.
If you get it and are fortunate/smart enough to get to the ER, they need to collect a culture to confirm the presence of the specific pathogen causing the illness. This will indicate what course of treatment (specific antibiotic) is needed.

Oh, yeah, I totally get that. I've seen plenty of things like "Pt denies history of chronic shortness of breath" and (on other charts) "Pt denies being pregnant" documented..
My notes said something like 50 yo male with HIV disease presented with pneumonia.
And then Pt denies history of HIV.
And I'm sure he's the one who added HIV as a diagnosis to my electronic medical record. If it was just listed in the notes, it wouldn't have been in my overall profile.
Getting it removed in one place was easy, but I still had a set of notes that any future provider at the same hospital (and the hospital is large system where I also get primary care, have seen a pulmonologist since, have had to see an audiologist) could review and would see "50 yo male with HIV". I didn't want to have to continually explain that or, worse, have ANOTHER doc see the notes and add it back again.
