R2, this is from fluctuating weight loss, and being overly strict about caloric intake in the neighborhood of anorexia at a young age. If you burn the “baby fat”collagen in your face, it doesn’t come back when you gain the weight back.
You can see the strain in his face in recent movies. I wouldn’t last ten minutes with a guy that didn’t have a hearty appetite!

Guy's body was delicious in Priscilla, but his face was at its most beautiful in LA Confidential. After that, he was really only sexy in that thin, druggie, trade kind of way (see: Memento). I'd gladly steal him away from the fraus, tie him up, and lick his body all night. He looked handsome in the first part of Iron Man 3.
