Oh, he had a reason, but the reason was 30 years out of date. There had been rumors very early on that Keanu was somehow responsible for River's death, so that's part of it.
But apparently Perry also says elsewhere in the book something like "why did Chris Farley die while Keanu still walks among us," using the same phrasing. To me, that sounds like he's just repeating the old 1990s jokes about how Keanu was a bad actor.
I wonder if the whole book is like that, full of references that would have made sense during the run of "Friends" decades ago, but seem bizarre today. Maybe he's been so screwed up for so long that he's like an old person with dementia, who can remember 1952 but can't remember yesterday.

Couldn’t he have some of the excess skin under his chin removed with needing opiate-type painkillers? Or medicate him but keep him hospitalized for two days and then send him to a health-centered spa (vs. actual rehab) for two weeks after that? I know alcoholics get surgery and take painkillers when it’s justifiable. Going without opiates after surgery might be uncomfortable for him, but looking at that is uncomfortable for everyone else.
Other than that, yeah, he looks older than his years, but not that bad. It’s possible that all the abdominal surgeries he had to have left scar tissue that makes exercising difficult.
I know it sounds ridiculous to suggest he NEEDS cosmetic surgery, but it his case the jarring unexpectedness of his appearance and his former fame are going to color people’s reactions to him for the rest of his life.

He has a weak chin to begin with and when he lost all his teeth due to his drug use his bones crumbled. Implants can prevent some bone loss but I don't think he got them.
'If you are subjected to bone loss in your jawbone, your face and mouth will suffer many consequences. The most obvious worry is how much your looks will change as a result. As the bones reabsorb, the excess skin and muscle begin to sag, giving you a very elderly look.'

In straight, front-facing photos his chin looked strong when he was young. But I think it was a little bit of an optical illusion. Even then he tended to have an undefined jaw with some fat deposits and straight in this photographed as a prominent chin instead of an incipient double or drooping chin. But at angles, the chi didn’t look at strong.
But the different is so dramatic, I have to think R41 has it right. Dental issues and bone loss contributed.
I say this as someone who also has, not a weak chin, but a short jaw. Weight gain does NOT look good on me. But I’m Perry’s age and I’m not seeing the kind of deterioration he has experience. That’s not average aging.

I don't know what an average 53-year-old looks like. Some look like Perry, some look a lot better than Perry, others look somewhere in between. He looks exactly like someone who, until recently (unless he's still using), was drug addled for most of his life. And he has the paunch of a 53-year-old who doesn't exercise at all--who literally drives to get his morning coffee. And he looks depressed. But honestly, he's looked like all that for years, at least to me.

Instead of stressing to make himself look hot for the comfort of many of you here, perhaps part of his recovery is learning to accept himself as he is now and prioritize his health and sobriety. As part of the ensemble, “Friends” was able to distract from how angry and miserable he is 24/7, sober or intoxicated.
Of course, the show made him too famous to ever blend into an ensemble again which put his awful personality front-and-center which the audience rejected over and over again (“Mr. Sunshine,” “The New Odd Couple”).
Hopefully his money and his rock-solid sense of superiority over all living things keeps him happy.

I don't buy that at all. Yes, there are plenty of alcoholics and drug addicts who had horrible childhoods and grew up with parents who were substance abusers, but there are plenty who did not. I don't buy the theory that if you're an addict, you must have had some psychological trauma in your past. I don't buy the reason he gives for his addiction in his book, that his mother was busy with her career and didn't have the time to give him the attention that he wanted. Some people are risk takers and like to try drugs and alcohol. I have the exact opposite type of personality and don't want to play with fire. I believe some people have more of a genetic predisposition to becoming an addict. They try something and it makes them feel good, so they use more and more until they can't function and their health is severely affected.
There probably are people who are chronically depressed or just feel uncomfortable in their own skin and they use drugs and alcohol to feel better. I know people with ADHD feel more normal when they take stimulants. I doubt any of these situations apply to Perry. I suppose it is possible that he was anxious about performing as an actor and a comedian and he turned to drugs and alcohol because they made him feel more confident. I don't buy that he became a severe addict because he had major psychological issues all his life or some childhood trauma.

I'm actually surprised after reading everything he's been through that this is the end result. The hair is fine, IMO, though he's losing it and I don't think he'd look good bald. But that can be dealt with. He can get fitter if he wants. His face - of the perpetually fat baby face variety - seems made to sag and get jowly. Hopefully he doesn't ever try to fix that.
