Is McDonald’s oatmeal healthy?

Without sugar, just the nuts and dried fruit/raisins in it.

I don't think so. They add so much sugar automatically it has to be bad for you. I actually had it this week when the cafeteria at work was closed for the day, and ate it without adding the cranberries and raisins. It definitely tasted a lot sweeter than the plain oatmeal I was used to.

According to McDonalds' Nutrition Calculator, the oatmeal before the cranberries/raisins/diced apples contains 15g of sugar. (Adding in the fruit pushes it to 31g.) I don't think there's a way to request it without the added sugar.

By comparison, plain oatmeal contains 1g of sugar.
