If he doesn’t qualify for any help under Medicare or insurance, how does anyone pay for care? I think R29 is correct, what he says is consistent with what we were told when my mother was dying.
I suspect that Forest has heirs that want to keep his estate intact (even if it’s small) and get other people to pay for the care. If it were just a matter of him being desperate to stay at home and wanting to decline funded institutional care, I’d be more sympathetic. So I don’t blame Midler for not taking care of it.

It didn't say what kind of care he needs (part-time or round-the-clock care) but if someone needs round-the-clock care and can't afford it, he would be better off in a nursing home, especially if he's near the end of life.
In California 20 hours/week in-home care costs $2513.00/mo. If he requires 24/7 care the monthly cost would likely be north of $20 grand/month. I doubt a go-fund-me is going to raise that kind of money.

Some points to clarify-
1- Forrest wants to be at home, and needs 24/7 care, which neither private insurance nor Medicare pays for.
2- He seems to be at the end stage of his life, which is why they are asking for $60K. That would fund 3 months of at home health care.
3- He has no children.
4- He absolutely would not get into the MPFT home in time enough. They have a large backlog.
5- Bette didn't offer any sort of monetary link in her tweet, so what was Lahti supposed to do? We only knew there was a GFM because one of us went and found it.

He may have been paid well for a few of his larger films, but actors see about 20-30 percent of their total pay if they're lucky (after taxes, agent fees, etc.)
He also struggled with drugs and alcohol so I assume some of it went down his throat and/or up his nose.

Most families of people who have to go on nursing home Medicaid plan far enough in advance so they can transfer all assets to someone else in the family. I have an old friend who died a year ago in hospice care after being in a nursing home for a few months. He had transferred everything, including the $800 thousand he'd received from the sale of large plot of land a few years before, to his son 2 years before when he became ill with cancer and was told he had no more than a year to live (which he beat by a year). His nursing home bill was $7200/mo. All the nursing home got from him was all but $50.00/mo of his monthly social security. Medicaid paid the rest. He was allowed to keep no more than $2000.00 in his checking account at any time to qualify for nursing home Medicaid.

There is or was Long Term Care insurance, but that market has changed over years. Companies soon realized policies written were going to take them to cleaners, so they cut back offerings, canceled plans, etc... Well, it seems the people looking into getting this actor the help he needs, seem clueless.
Many wealthy and middle class people are actually eligible for at home care which is covered by Medicaid. Wealthy and middle class people can, and do, legally move their money and other assets (most wealthy people and middle class people usually own a home, a co-op or a condo) into a trust and then are easily eligible for managed at-home care.
A relative, a millionaire, has been in one these programs for about 3 years. These programs cover the home aides care and any needed supples. When I see these GoFund me pages for people who need this level of care, it seems the family and even friends of these people do not know how to navigate government help when it comes to medical issues.
In fact, when my father was briefly in a nursing home for re-hab after surgery, one of his roommates was a man who owned four apartment buildings and his own home, the man simply transferred all his assets into a trust for his three children. This man was on full Medicaid.
In fact, you don't even have to be on full Medicaid to get at-home care. If you are retired and was able to carry on your health insurance and have Medicare, Medicaid, will cover the at home program. You other insurance plans usually cover the rest, again, it depends on your plans.
If a person's SS and pension are over the ceiling monthly amount allowed for this type of care, they might have a small amount to pay for the care, say around $500 a month. This is much less than paying $4000-$5000 a WEEK and that's just for the home aides, especially if the person needs 24/7 care. At homeland term care sure beats being in a disgusting nursing home.
Eligibility for these Medicaid covered programs also depends on what state the person needing the care lives in. Many red states have not expanded Medicaid.

Seems those Medicaid reimbursement laws sure don't apply in NY state, especially for a Medicaid recipient's relative who inherits less than one million. Most of these people who put their money and assets into trusts need to hire an eldercare lawyer who helps them navigate the system. Believe me, wealthy peoples families are not reimbursing Medicaid. At least not in NY state.
The only fee a person would have to pay is they have monthly surplus. In NY state, a Medicaid recipient who has at-home care covered by Medicaid, cannot bring in more than $900 month. For a fee of about $50 a month, you must join a pooled trust. The Medicaid recipient, or their relative, will tell the pooled trust what bills they want the trust to pay. The pooled trust then takes the amount out of the person's checking account to pay the specific bill or bills. The pooled trust has nothing to do with setting up a trust which contains the person's assets, the pooled trust is completely a separate situation. This a lot to navigate and if the elderly ill person doesn't have a family member to help, things get dicey.
Do you realize how many people are on Medicaid in NYC? Not just the elderly in nursing homes and those receiving at-home care. Many of these New Yorkers on Medicaid work off the books and are walking around with $300 sneakers. Don't get me started.
One of my relative's home aides is on Medicaid herself! Not sure how she manages this, as she gets paid $20 an hour., she earns around $1000 a week, way over the ceiling amount for Medicaid coverage. I have to wonder, is the home aide agency paying this aide half on the books, half off? Lots of these home aide agencies are extremely corrupt and shady, for instance, homes aides are not allowed to work more than 40 hours week, this aide works over 40 hours. Most of the home aide salaries are covered by Medicaid. These agencies depend on Medicaid to survive.
This aide, with overtime, actually makes over $50,000 a year, has an expensive iPhone, has a few LV bags and well, she should not be getting Medicaid and food stamps, but she does.

What I posted has nothing to do with spending down one's assets. I do know, there is now a longer look-back period for ill people who need permanent nursing home stays. It's now about 5 years. look-back on all assets. For at-home care, there was formerly only a one month look-back for at-home care programs. Not sure what is now.
Covid sure changed Medicaid laws, especially in NY state. Medicaid laws in NY became more lenient. For example, people on Medicaid, who only have Medicaid for their health insurance and are not in any kind of nursing facility or have at-home care, were automatically reinstated and didn't have to fill out their yearly renewal forms.
Personally, I don't know one person, who inherited a relative's estate, that had to reimburse Medicaid for their late relative's care.
Also, since most healthcare plans only cover up to100 days for nursing home care, smost nursing homes apply for Emergency Medicaid for their patients. These patients obviously weren't smart enough to move their assets before they needed this level of care. The nursing home social workers step in to apply for Medicaid for their patients who need to remain in these nursing homes.
Those linked Medicaid articles do not seem to apply to NY state residents.

Philanthropy is one of the largest and best deductions wealthy in USA and many other countries have to lower tax bills including final estate taxes.
They may not give to causes that benefit charities of your choice, but wealthy do donate a bulk of such funds including setting up foundations and trusts.
Mr. Jeff Bezos recently announced intends to give away nearly or all of his fortune before he dies or will dispose of same via various mechanisms after death.

SAG-Aftra does offer health insurance.
Years ago, a friend joined because he was hired to be in a few TV commercials. A friend scouted him due to his looks and personality. My friend was not an actor, he was a fashion illustrator. He kept up his SAG-Aftra dues in order to keep his health insurance. After he secured a staff job as an illustrator, he stopped paying his SAG-AFTRA dues. He had decent health insurance through his job, he knew he had no desire to act, so he dropped his membership.

It isn't clear FF asked anyone to do anything. Helen, Barry and Elliot acted on their own it seems on behalf of their "friend" Frederic Forrest.
As stated many times in this thread we none of us know entire backstory.
In many cases a hospital will not discharge someone who is ill (especially terminally) who cannot live on their own. Nurses, social workers and physicians will make that call. If someone cannot be sent home most likely two options are being sent to a skilled nursing/rehab facility or kept in hospital (long term border).
Mr. Forrest is unmarried and childless. Without family or friends willing to provide 24/7 care he needs to live out his days at home, it likely won't happen unless people are hired. Hence the GFM page.

Whether or not SAG/AFTRA offers health insurance wasn't the question, we all know it does. In regard to FF query was does the man still qualify for benefits having not worked since 2008 (film "All The King's Men").
At age 85, Mr. Forrest would likely fall under "Retirees (Including Senior Performers". See link provided in your post on how that works.
Like nearly everything else involving healthcare benefits it isn't very straightforward, but much of it comes down to whether or not said senior/retiree has enough qualifying Eligibility Threshold (currently $25,950) in Covered Earnings during their Base Earnings Period.
As others have stated in this thread SAG/AFTRA benefits are largely tied to amount of work a performer does within a given period. No work and if that goes on long enough, no or low benefits.
Recent Covid pandemic and various federal/local laws regarding vaccinations have put performers out of work. If vaccination is required to work on a production and performer doesn't want or have, they cannot work. No work - no bennies.

I remember Fereric Forrest from "The Rose. He was good in that; he always seemed to me to be a competent, if ordinary, actor. His career never really took off, though he and Teri Garr starred in a bloated production by Fances Ford Coppola called "One From The Heart." They played an unappealing couple named Hank and Frannie and the movie is about their failng relatitionship and aimless lives. Coppola was gvien free rein to do what he wanted and he raised the budget from $15 million to $23 million. As a result the movie is very "stylized" but has no depth or substance. The movie was originally supposed to be a romcom, but what the hell did Coppola know about romcoms, so it became a more "ambitious" project. Anyway, the movie was a colossal critical and financial flop. and it did absoulutely nothing for the careers of Forrest and Teri Garr. They wer miscast anyway; both of them were much better supporting players than leads.

Even if you don't reach your goal, you can withdraw every cent, minus the transaction fees. This is unlike other donation platforms that take an all-or-nothing approach, meaning you can only receive the donations if you reach your target.

I hate GoFundMe. Remember the sob story about the homeless man who gave a couple his last $20 so they could buy gas? It netted over $400,000, ostensibly for the dear homeless man. But it was a rackett; ; the couple and the supposedly homeless man made the whole thing up. I woould never give a dime to any GoFundMe account. But there are always stupid people willing to give their money away.

Someone finally made another substantial donation ($6000). Total is $56,670 raised of $60,000 goal.

What's the movie where he was playing someone "slow" and he went to pay for something with a large bill. The cashier asked him if he had anything smaller. He took his cash out of his wallet and laid the bills next to each other and replied "No, they are all the same size."
