Too much sun in his youth compounded by some expensive cosmetic procedures. Avoid the sun and moisturize starting in your teens is the best way to prevent this.

His cheeks were always jutting out like that, but now there are jowls below. Weird that his neck looks even redder and more weathered than his face. I'm guessing he had a facelift and neglected to do the jowls & neck.

He's always had practically zero fat on his body. One of the leanest men I've ever seen. Men that lean don't usually age well. There's nothing under the skin to keep the wrinkles from taking over. No padding. How he and fat Randy are brothers is perplexing. I guess it's possible Dennis starves himself that thin.

Could be rosacea. Guy i know suddenly developed it when he was about 60. He can no longer allow sun to touch his face or it goes all red, lumpy and swollen.
