People in these societies - and Iran is just one of them - believe you're not gay if you're the top.
All gays are bottoms to serve men's needs. They are substitute for women. They are no better than women.
They deny that gays have emotional lives and psychological needs that bond them with other men and not with women. (And let's not even get into what they think about women. They are not even allowed to smile in some of these societies.)
