Isn't there a line in the film that says he was not a true pre-op trans, he just thought he was? They probably threw that in there so there wouldn't be protests.

I don't remember if it is spelled out in the film, but the book makes it clear that Buffalo Bill believed himself to be transsexual, but that the doctors that he went to believed that he wasn't, and denied him sex change surgery. Of course, by today's rule, his belief that he is trans would make him trans. Unless, of course, the trans community wanted to distance themselves from his atrocities, in which case they might declare him "not truly trans." Or, they might blame cis-gay meant for driving him over the edge by not respecting his identity.

The "Would you fuck me? I'd fuck me." scene in the movie makes a pretty strong case for autogynophilia in the movie version of Buffalo Bill.
