As a child of the '90s, I spent a good deal of my youth at video stores. This one in particular terrified me—it was years later that I finally saw the movie (which is gloriously campy), but the video art was the stuff of my childhood nightmares. I was also freaked out by the famous lips cover for "The Rocky Horror Picture Show" (not sure why).
I don't know if this was common in other video stores, but in our local one the horror movies would be placed right next to the adult movies. So as a young kid I'd be going in with my parents to get something family friendly, and eyes would stray over to the horror covers, which terrified me but I couldn't stop looking at them, and then I'd look across more and see naked women everywhere. Most people nowadays will talk about the connection between horror and sex in film, it's interesting to think back and see how literal that was in my childhood.

^Oh and sorry, to actually add to the point of your thread, OP, I remember being freaked out by the cover of the Ghoulies movie, with the ghoulie coming out of the toilet. This was also the first horror movie I ever watched, my father let me watch it with him, but I would've been like 6, 7 years old? And he'd let me drink small sips of beer from his bottle.
To this day I love horror movies. But barely ever drink.

If album covers count, I used to see Motley Crue's "Shout at the Devil" in stores when I was a kid, and it made me feel uneasy, even though I didn't know what a pentagram was.
Of course several years later, it became one of my favorite albums at the time.

The covers of 1974's Its Alive always brought back memories of seeing the mini trailer on TV as a kid. Supposedly much better than the actual movie. So scary in a dark room when you're 8 years old!
