Seriously, there is NO way that was an accident. Someone thought it would either be funny and clever or more likely, they thought they could slip it by people and have a private laugh.
Everyone who reviewed and approved it should be fired, not because I'm outraged about the "insensitivity," but because they jeopardized the brand and offended customers. Whoever is responsible cost the company a lot of money, no less so than if they had launched a failed marketing campaign (which this was) or poorly selling product.
