I've heard this a few times as in "Who all is going to the party?". Is this an American regionalism and, if so, what region(s)? I think the people who I heard say were from the lower Midwest (Missouri or Kansas).

From Maryland and I say both “who all” and “you all.” I never say “y’all” and I find it annoying when non-Southerners try to adopt “y’all” and act like it’s natural.

Sounds normal to me. The second person plural equivalent is "all y'all." "Y'all" is a polite form of address and it grates on my nervous when people use it in an impersonal way.
