R2 what would that have done? Just because Bale can lose weight and look fine after doesn’t mean Matthew will have the same effect.

He's 50 years old now! I know from experience, your face looks different than what it did in your 20s and 30s. I look in the mirror and I see two of my grandparents looking at me. Trust me, it will happen to you, too.

As pointed out above, r50, that's due to the extreme weight loss he did for the DBC movie. But at least he got an Oscar out of it, so there's that.
Plus, I think for many, if not most, male actors who first make it big because they're considered handsome, losing (or destroying) their looks is something they they think will raise their stature, making people take them more seriously. Look at Di Caprio, he's an extreme example. I don't get the feeling either of them mourn their lost looks.

I call some sort of illness /medication combo on this. He looks incredibly bad in the Doritos commercial and that's with good lighting and digital enhancement. Not just old, but like something is wrong. Also getting stoned on a regular basis does catch up with you at some point.

When you're 50 and determined to go around shirtless showing an eight pack, you're going to have a hollow, sickly face. It's inevitable, unless you get a face full of fillers that make you look like you have a subdermal infection. Any attempt to defy age at some point looks like illness.

I'm a McConaughey on my mom's side. I got thin at the beginning of quarantine and looked like death, even just 10 lbs makes a dramatic difference. We're rangy, long Celtic (?) people who need a certain amount of body fat- see Liam Neeson in that Scorsese monk movie, Christian Bale all the time, and I'm sure there are others I can't think of. We make lanky warriors who can quickly climb the clifts of Scotland and Ireland... I assume. But can look drawn very quickly.

I'm in my forties and when I look in the mirror now I really see my father. It's rather startling how much your looks can change as you age.
