It’s very sad. I’m glad the wife tried something to help this complete bi polar meltdown. I’ve never followed any of them but this past week it was unavoidable. I have a friend who’s mother is bi polar & when she’s off her meds all hell breaks loose - and she’s not a celebrity with major bank.

Unfortunately the pendulum swung too far to the left when they reformed mental health rules/system. That's why so many mentally ill people end up homeless and living on the street. Unless they pose a danger to themselves or the public, they are often left to their own devices and end up in a downward spiral. Finally, the cost of warehousing folks for decades or for many months out of a year was an increasing financial burden. The original reforms on the federal level called for the establishment of a robust community mental health system, but it was never fully funded. Beginning in the Nixon years, there was an over emphasis on cost-containment that resulted in an underfunded community system that could never handle the release of patients from the hospitals that’s were largely state funded. And states didn’t invest either. Legal reforms that emphasized competency and patient rights were indeed needed, although the reality is that legal competency standards can be met much quicker than full insight and long-lasting recovery from a chronic mental health condition.

I knew a young guy who started doing this on Facebook. I knew he was struggling personally, but all of the sudden, he disappeared and when he returned to posting, he had shaved his head and looked like he had been in a fight. I think he was even missing a front tooth. He started posting all these random photos and strange selfies. He considered himself a musician, but would post these "songs" that were just a bunch of racket and him screaming or making strange noises.

I contacted a mutual friend out of concern and he was in touch with the family. She said he was diagnosed as bipolar and schizophrenic and they were trying to get him on meds, but he didn't like taking them, which would lead to manic episodes. These episodes were what I was seeing online. The stuff Kanye is posting on Twitter and his Whoopity-scoop, whoop-poop stuff reminded me of this dude.

Reagan didn't close the mental institutions. He defunded community based services. Actual hospital closing began under JFK and continued over the years with assists from the ACLU and several health and disability groups. Learn the history of deinstitutionalization. It's long and complicated and an example of how hood intentions have unintended consequences. FYI...the ACLU, to this very day, fights to keep homeless on the streets even in sub-zero weather.

Some of my psych patients like the highs of full blown mania. The more devious ones will fake depression in order to me to prescribe antidepressants and stop taking their bipolar meds altogether. Most of the time it’s a combination of reasons for these manic episodes, like missing meds or increase stressors. Once the mania appears some actually enjoy it so they skip meds entirely.

Kanye IMO is bipolar as opposed to schizoaffective disorder. There is a distinguishing feature of schizoaffective disorder from bipolar disorder. A patient with schizoaffective disorder will have symptoms of psychosis even if the labile mood (depression, hypomania, mania, rapid cycling) is absent/ managed.

He’s more likely bipolar with psychotic features. Someone with that diagnosis only experiences symptoms of psychosis with worsening of their mood, and this includes not just manic episodes but depression as well.

However, no the body can’t tolerate it for long without decompensation. Particularly with the brain, manic episodes are linked to the loss of gray matter in the brain. This may explain why people with bipolar disorder have a much higher risk of developing dementia. This is due to the cumulative effects on shrinking of gray matter.

I have a theory that those with very stigmatised diagnoses, such as a Multi-Axis diagnosis with something very dark, probably will not disclose the full truth of that. I have known individuals such as ths; one with both OCD and Schizoaffective disorder. He only mentioned the OCD, then when he was really in knee-deep shit, his family members told me about the full diagnosis, and that he was taking an antipsychotic medication.

Bi-Polar disorder has enjoyed much exposure as of late, with many brave sufferers coming out about their experiences coping with the disorder. This has surely helped de-stigmatise the label. When people think or hear of Schizoaffective Disorder, they automatically think Schizophrenia, (which is wrong) and there is a much higher price to pay for announcing to society you suffer anything "Schizo".

This illness often goes hand-in-glove with those exhibiting the paranoid type of Bi-Polar disease, characterised by long manic intervals. It often goes unchecked until many cycles of mania are observed. Continued impaired communication and speech, and extended bouts with mania lasting two weeks or longer are hallmarks. Other Bi-Polars cycle different to these, and manic episodes do not usually persist as long ss Kanye's.

Just because someone admits to suffering one malady does not preclude others. It makes sense why some wouldn't admit this, even if they had been diagnosed. Schizoaffective Disoorder occurs a little more than a third as Schizophrenia, but does comprise nearly 30% of all inpatient admissions to hospital for psychosis.

Two or more presentations within a one month period is one of the "Criterion A" for consideration. Kanye's behaviour most definitely reveals that. One would need to rule out drug or drink as a catalyst, however Bipolar-Type is the most frequently seen over the Depressive type.

He ought to be administered an Antipsychotic, and observed if it helps level him out, and reduce his voices and delusions IMHO.

What we do is we go from the class and type of meds that are most effective but have the least amount of side effects or at least side effects that are tolerable. If that doesn’t manage the symptoms then we move on to second line meds, so on and so forth. These meds as we move further down the line get more toxic and laden with undesirable side effects even as they made be more effective at managing symptoms.

That’s why it’s up to Kanye and patients like him and family members to be truthful about whether they are adherent to taking meds as prescribed. Because if you lie and say you’ve been taking meds and they’re not working, I’m thinking you need to be put on another class of meds that may have more side effects.

This is a myth.

In 1967, the California legislature passed the Lanterman-Petris-Short Act (LPS), which allowed privately-owned mental facilities to accept more low-risk patients vs. incarcerating them in publicly owned facilities with violent patients.

Governor Reagan signed the bill into law as it was clearly a win-win: mental health patients would receive safer, more cost effective treatment, and in turn the funding to state mental facilities could be reduced since the population would be reduced since there were less patients and the safety profile changed.

Predictably, the state facilities freaked out about the budget cuts, even though they were not underfunded because budgets were based on headcount. Which had just shifted to facilities better for the patients and more oriented toward low risk treatment. So, the state hospitals reduced some headcount after complaining bitterly.

Despite the bipartisan plan, unforeseen consequences developed. Without a legal requirement that all mental health patients go to a public facility, many borderline simply exercised their right to refuse private treatment. The upshot was that many individuals who could have benefited from treatment chose none at all, and instead attempted to function in amongst the normal public.

Later, the Carter administration (not the Reagan administration) signed into law the Mental Health Systems Act of 1980, which largely promoted the same idea for national facilities.

In 1981, both parties in Congress agreed to the annual Omnibus Budget Reconciliation Act of 1981 (which is must-pass legislation, like the NDAA), and President Reagan of course signed that into law. He had no more choice about this than any other president.

One of it's many provisions was to eliminate federal funding for community services and thereby transfer funding back to individual funding or state-funded efforts. Had Reagan even been aware of that part of the Act, he would have immediately realized the Act was going to replicate the disastrous effects of the LPS he had experienced at the state level in California. And then had to sign the thing anyway.

In 1986, a bipartisan attempt to correct the problem was made in the Mental Health Planning Act, which created a funding pool of federal money for Medicaid to help serious cases. But it was too little, too late.
