Looks like rheumatoid arthritis. My dads fingers look exactly like that when he has a flare-up. It's an autoimmune disease that is fairly common in the elderly, and treatment is symptomatic. It can be extremely painful.

Another vote for edema, which is usually a sign of congestive heart failure, but given how long-lived his immediate ancestors are, I somehow don't see Chuck dropping dead in the next few weeks.
So maybe it is rheumatoid arthritis.

There is a type of arthritis called spondyloarthritis that causes an overall bloating and inflammation of the hands unlike rheumatoid arthritis that usually causes swelling and inflammation in the knuckles and the finger joints.

Looks like the classic case of psoriatic arthritis. I used to see patients with it time to time at the rheumatology clinic where I did part of my training.

Also from that photo of his feet, if those are his feet, he appears to have nail psoriasis, which is linked to poorer prognosis in psoriatic arthritis. Some patients also have facial rash or ruddiness on the face.

This is a systemic problem and more than likely a form of arthritis. R 29, R31 and R32 suggested types would be part of a differential diagnosis, but more information is needed to make a definitive diagnosis. Having this type of condition involving ones digits can be debilitating.
There are successful biologic treatments for the conditions associated with Psoriatic Arthritis. Which makes me think that this make not be Psoriatic Arthritis.

I really hope you're wrong r39. I sometimes think that William is going to be lonely when he gets to the top. Queen, Charles, Philip, (not in that order), Harry (MIA and having his own existential crises), etc. That whole upper tier will be gone sooner rather than later. Swelling of both my hands similar to this led to my diagnosis of rheumatoid arthritis. I have been on methotrexate and Humira and it took almost 6 months for the swelling to go down. My hands peeled after they deflated. I have lived a long time and this is the worst thing I have ever had. I feel for him. Its very painful.
