I watched the entire clip and you can clearly see the underwear in the last few seconds. It is hard to tell from a distance.

I've seen a fake prosthetic penis prototype up close in a makeup effects shop and they are made of latex. It looked and felt like a super thick condom that an actor would roll onto his dick. If the prosthetic is to include a scrotum too then they make it with balls and there's room for the actors balls inside, but the one I saw had a thin flesh colored string that that worked like a g-string to assist in keeping the actor's balls inside the scrotum part. Then there was a fake patch of pubic hair. It was interesting to see and one of the guy's in the shop was the penis they used to make the mold for the actor. So someone in the shop had a huge dick and balls, but the Makeup guy wouldn't reveal which guy it was. LOL.
