Paparazzi stalking celebrities except in very few A+++ cases is fiction. Also many celebrities stage paparazzi photo ops expressedly so they can have a true private life. If a star gets caught doing something there’s usually an accompanying substance abuse problem ala Ben Affleck.

Private estates and blacked out vehicles. There's a way for everything. And well paid entourage who keeps their mouths shut.

A lot of time there is also an understanding between their spouse that they might have six outside the marriage. It's part of the acting process to get know your leading man for instance.
