The guy you're referring to is James K. He seemed pretty hopeless. He'd be on a strict diet but would whine to his idiot girlfriend that he wanted something else to eat, and she'd relent and order in Chinese or something else equally off limits. Whenever he was transported somewhere he'd moan in pain and act like he was being killed. The doctor on these shows is named Dr. Now; Jame K. would show up to be weighed and of course he'd put on dozens of pounds and they'd give him excuses like the car had broken down so the girlfriend couldn't buy food do they had to order food to be delivered. The last episode featuring James K. had Dr. Now telling him he had to go back where he came from; if he lost weight then they could discuss the possibility of weight loss surgery. James K. seemed very upbeat and was sure that he was going to improve. Viewers have been curious about what's going on with him, but no follow up about him has been scheduled.
The disgusting blob the OP mentioned is named Steven (or John) Assanti. He and his brother Justin were both featured on "My 600 Pound Life." EVERYBODY on these shows blames their food addition on being abused as a child and these two are no exception. Justin is the more sympathetic; he seems to have always been in the shadow of his big brother Steven, who gets all the attention. Steven, as some viewers have suggested, appears to be a sociopath. His behavior is chilling; he's a pathological liar and manipulator and if doesn't get what he wants he hurls abuse at anyone around until he gets it. His videos on YouTube are further proof that he's a crazy man, irredeemable, a vile piece of garbage. God knows what he'd be doing if he wasn't a bedridden behemoth.
There's going to be a follow up to the first show about the Assanti brothers next week. I hope the poor brother Justin seeks help; Dr. Now offered him weight loss surgery, but he declines and went back home with his fucked up father. At the end of the first episode the horrid Steven was sent to drug rehab (he was addicted to painkillers). Why doesn't the numbskull Dr. Now just tell him to fuck off? He's probably the most loathsome character ever to appear on this show. I think a lot of viewers would like to see him explode like Mr. Creosote in "Monty Python's Meaning of Life."

I was only able to watch about 12 minutes from the attached clip before I had to shut it down. Heartbreaking. A doctor answered that question on one of the shows and his answer was this: by eating. Eating lots of food high in calories and never getting an exercise. Of course people who do that blow up and eventually become immobile. So they have an enabler who goes out and gets the food and brings it to them.
Everybody on these shows always say they started overeating due to some kind of childhood trauma. That's always the excuse. But I remember at least on case where there was no childhood trauma. It was about a teenage boy named Billy Robbins who had eaten himself to close to 800 pounds. In that case it was his mother who had experienced the trauma; she'd lost her first son Matthew while he was still a baby. Some years later she had her son Billy; she doted on him, pampered him, spoiled him. Billy Robbins had a father, but the poor man appeared to have been shut out of his son's life by his wife's smothering attention to her son's every need. The relationship between mother and son was truly sick; she would refer to him as him "my baby" and spoke to him like he was an infant ("You hungies? You want Mama to make you a sandwich?"). Anyway, Billy Robbins was similar to Steven Assanti; he would whine and complain until he got what he wanted and even physically resembles him. They did two shows about "the half ton teen" Billy Robbins; at the end of the second one his fate was uncertain. He'd had some weight loss surgery (they sliced off mammoth slabs of fat from his blubbery body) and had lost weight but was backsliding again. I think they didn't do any more follow ups on him because he's ceased to improve. He's probably still living with his doting, besotted mother. I can't say I feel sorry for people like this. It's their life, their choice.

Really the best, most effective treatment would be for a judge to put a restraining order on the enabler family members. Keep them separated from the morbidly obese patients and assign home health aid workers to bring the fatsos their food rations. The workers wear these gloves, but then I've seen them on shows like these having to go deep into folds on the patient's bodies... and the gloves only go up to the wrists. Most doctors would refuse to perform surgery on someone that obese because it's just too risky. The patient is required to lose a certain amount of weight before surgery to lower the risks involved, and to prove that they are really serious about losing the weight.

It's still heartbreaking -- and painful to watch. I am 50 pounds overweight (post giving up smoking) and hate that enough, just in terms of stairs and clothes and some general aches and pains that come with getting older. I can not even imagine struggling to rise from a chair. And thinking of the organs working overtime underneath all that weight... yet they're still alive.
I thought the same thing when Darlene Cates (I think?) died, the mom from "Gilbert Grape". She made it to age 69 at that weight. So maybe the body has amazing tenacity even under those circumstances.

The poor medical staff who have to take care of their pigs should get gold medals. These huge piles of blubber have to be cleaned like an infant; the show actually films this happening, blurring out certain areas of course. One show depicted a mother lifting her tree trunk leg up so her daughter could clean her between the legs. Another show had the viewer treated to the sight of a woman whose boyfriend (HOW can anyone become involved with someone like this?) was cleaning her butt crack. It's revolting, yet it's hard to look away. It's just so unbelievable.

R103 - what can be done to help these people? To get that big they all have SEVERE psychological issues. The only way to stop them eating is to remove them from their families and force them to stop, and the doctor doesn't always have the right to do that. I've only seen a few episodes where he was able to commit them to the hospital to be on a controlled diet... and even then their parents/lovers snuck fast food inside.
It's like treating anorexia, they will actively go behind your back to avoid treatment, there's only so much a doctor can do.

[R113], I suspect a significant number of the viewers watch this show (and others), and say to themselves, "At least I'm not that bad." Others view the show while on their treadmills.
One thing is clear, when I hear the poignant piano/violin music, I know I'll get to hear about the loss, abuse, or suffering these fine folks endured. Thing is, if one lives long enough, all of us will experience loss, abuse and/or suffering. There's no getting around it. So, I admit to having mixed feelings about those, virtually inevitable, portions of the shows like these (I include shows like Hoarders, ect.).

I'm attempting to understand how people get here. I'm guessing it starts in childhood, and the weight gain just continues from there.
I grew up with a mentally ill mother. She was horrible, still is. I'm somewhat grateful that I coped with drugs and alcohol, because I can't imagine coping with food, and ending up disabled due to my weight. Getting sober is no picnic, but it is when one sees the catastrophic effects of obesity. Obviously, it's easier to stay sober, than to lose weight.
Steven is a cunt who should be left in a locked room with nothing but water, a treadmill, and fruits and vegetables for a year. What an asshole.
Watching this has given me a lot of compassion for people who go through this. I hope that anyone struggling with this can overcome it, and find happiness regardless of the outcome.

I remember seeing one of these shows a long time ago where this obese young guy lost all the weight and got into shape was transformed into a good looking young man that young women were suddenly interested in. You can imagine how thrilled he was.
Really amazing. But I did wonder if in a bathing suit his body would be covered in scars due to the years of stretched out skin and it being cut off.

All addictions are about quelling anxiety. Food, liquor, drugs.... They all numb or distract from anxiety. If we can figure out how to treat anxiety without numbing other parts of brain function, we will have solved addiction. Not easy or straightforward though. Recent studies have suggested we developed exagerated anxiety as a by- product of evolving larger brains/cortexes. So we are smarter but a percentage of us are nervous wrecks. How to help those people? That is the issue.

Do you know what helps get rid of anxiety? EXERCISE. It's been proven that exercise is a good way to relieve stress. But most people want to do something that doesn't involve much effort on their part, like taking a drink or a drug or eating. Most people are lazy. They do what's easy, not what's beneficial.

Anxiety's (using a general and non-technical meaning for the term) role often is present. However, addiction often, if not usually, is much more complex than "I am trying to get okay with my own head and face things through my taking X."
Biological bases for addiction, the role of such substances in pleasure seeking, the social component of addiction, and the fact that people are not just anxiety machines need to be considered when dealing with addiction. And the absurd notion that "EXERCISE" is the answer is just as ridiculous as saying that alcoholics drink to avoid anxiety. Is everyone so untutored and tweet-brained that easy answers are all they can admit for complicated human issues?

I have suffered the highs and lows of depression my whole life. Genes and terrible terrible disappointments. They can trigger some horrible feelings in me where I literally don't want to live.
Exercise personally has meant nothing in terms of ridding myself of terribly sad feelings no matter how well I get my body into shape to the point where people think I'm an athlete. And god knows I've never been an athlete.

These folks are stretching the limits of what I previously thought possible - just a few years back, I would have had a difficult time believing that a person could get that large, and live.
Adipose tissue (fat), is not an inert substance. It is quite active. It interacts with the body in all sorts of ways, and when excessive (or too minimal), the entire body system is thrown out of wack.
I cannot even imagine what that amount of excessive fat is doing to the endocrine system, for one thing. I have wondered if that explains some of the behavioral and emotional problems I have often noted in very over-fat people.
Also, many of these people have never known what a normal amount of food is - never in their lives have they experienced eating normally, for any extended period of time.

There are not an abundance of medical facilities in the U.S. adapted to treating 600 lb. patients. That hospital in Houston is very reputable, and have accepted patients from all over the country. Dr. Now is well credentialed. The show only presents a fraction of the assessments and processes involved. Many of these patients are also referred to dieticians, physical therapists, trainers, and counselors as part of treatments. The show depicts some of that.
These patients are not just a challenge physically, but also carry much psychological baggage, addictions, negative attitudes, destructive habits, and accompanied often by complicit and confused enablers.

It's interesting that I was formerly such a lush with booze, because I am extremely disciplined in other areas of my life.
I LOVE LOVE LOVE eating healthy, whole, home cooked meals, and I truly enjoy exercising. I have since I was a teen, and even when I was in the throes of addiction, I made it a point to eat sensible and nutritional meals, and I made a point of getting out and at the very least, walking around the neighborhood for an hour. It actually gave me hope that I would eventually beat alcoholism. I'm also extremely disciplined with work and was even more disciplined with school. My mother was a fucking mess who never taught me anything of value, so I'm not sure where that personality trait came from, but I'm grateful that it is there. But once active addiction enters my life, the less disciplined I can become, and it can spiral out of control. Had I had enablers, it most probably would have. But I don't. I've always had to pay my own way as an adult, so I had to join the rat race with everyone else, despite my addiction. Yes, addiction wreaked havoc in my life, but it would have been far worse, or perhaps fatal, had I had an enabler in my life.
These people on this show ALL have very sick enablers surrounding them. They had plenty of malevolent assistance when it came to this type of weight gain. In order to succeed, they will probably have to cut these enablers off. I don't see any other way around it. Even if the enablers get treatment as well, it's just way too tempting and easy to fall back into familiar roles and dynamics that are antithetical to weight loss.
My heart truly goes out to them. It is not OK to judge people in this situation. I absolutely judged the overweight, which was gross ignorance on my part. After watching several of these episodes, I will never judge obese people again. It's just not OK to laugh at others who are obviously in lots of emotional (and physical) pain. I feel like a complete cunt for ever having done so, and it's a horrible trait of mine that I need to change.

I think it was mentioned on an earlier thread about this topic that anyone that cares for the obese person (boyfriend, mother, etc) gets some sort of compensation. Also, when you're so obese that you're bedridden, you're not paying for transportation, or for things like clothes and nights out. so a lot of your money could go to cheap food.

You can tell though that most of them have poor nutritional knowledge and are poor (shown by the weight of their family), but have a mental disorder on top of that to take it too far.

If he's receiving any type of public assistance, the providing agency should require his weight checks be done on truck weight scales which are on most interstates or a city zoo where they weigh hippos and elephants. These are govt funded facilities and his time on the scale should be open to the public. A few public shamings and his manipulative self would drop those pounds. Once he reaches 500lbs., his weigh ins could be done at the surgeon's office.

That type of weight gain goes beyond metabolism, thyroid, or prescription drugs issue. This is rage eating. Some people call it emotional eating or emotional binging, but I believe it's rage.
Steven is an extremely angry man. He hates everything and everyone, and he also hates himself. I'm not sure what his background is, because I haven't watched this show other than perhaps twice in my life. However, it is obvious that something went terribly wrong on his childhood or adolescence, in order for him to get this way. Obesity is a manifestation of severe depression.

According to the show, Stephen's mother is/was an alcoholic. After his parents divorced, Stephens mother attempted to raise the boys. She often left them alone to go out to party and drink. She'd leave some food out for them and then be gone for hours and even days. Justin's hatred of his brother began when Stephen would eat most of the food the mother would provide, leaving Justin with little to eat.

Well, that explains it, r207. So eating was a way to cope with the emotional loss that a child feels when neglected, so he ate all the food as a sign that he, himself, was loved, and his brother was not deserving of it. Which reinforces my opinion that Steven is a sociopath.
Also, I haven't ever read up on a link between obesity, and Fetal Alcohol Syndrome, but perhaps there is one.

I watched some of the video supposedly showing the fat fuck Steven Assanti eating his own shit. It's a fake. He reaches behind him with a tissue or something and holds it to the camera and says "as you can see, it's legitimately shit and I am about to eat it", then starts making fake noises like he's about to throw up. I stopped watching after that. Since the viewer does NOT see him wiping his ass with the tissue, the "shit" is no doubt chocolate syrup or chocolate cake icing or some other brown substance that is NOT shit. The fat fuck just wants attention and will even resort to making a video where he's supposedly eating his own shit to get it. Sick fucking bastard. Aside from his manipulation and psychosis, it is sad. He is just one of those extremely obese people that refuse to change their eating behavior, and deceive themselves that they plan to change. They are full of excuses and and will project blame.
About 5% of the morbidly+ obese will actually lose enough weight to function normally and not gain it back. Some of the younger ones will survive a few years longer if the heart is still strong enough.

On part 2 episode Dr. Now told him if he tried to get drugs through his usual manipulations he would be arrested, so he's up shit creek as far as feeding his habit goes. The same episode showed him actually losing some weight. Is TLC really going to continue featuring this very hated individual? If the shows keeps getting ratings, I guess it will.

^R228 You have a point to a major degree. A judge may determine the prison system is not equipped to handle someone of that size and physical needs. Also, giving him a fine may be useless since he is unemployable. With his mental issues though, he could be mandated to a state psychiatric prison for a short period. This is Houston and Texas, and they may have such an option, but it would be a risk and strain also. More likely, he would be sentenced to house arrest, allowing him to leave for treatments. No real difference since he is basically bedridden anyway. His probation officer would call and visit periodically, and could try to take urine samples or have it done when he has hospital visits..

My gawd. I can't believe a person like this actually exists. I've had my bouts of depression. I almost drank myself to death and gained tons of weight during that time (55 lbs), but I rebounded. I just got sick and tired of being drunk and fat.
There are plenty of reasons to let one's self go. I get it. The world isn't some rosy garden, where everything goes our way and happiness abounds. But fuck, once I realized that reality can and does at times suck, I remembered that there are also good times. Even if rare, I'll take my chances by waking up and facing the day as a healthy, slim, middle aged wanker. It beats the alternative, you know?

The clumsy editing irritated me. You could tell from the changes in physical appearance (darkened tooth, hair) that the show was put together out of sequence and events like toppling out of the golf cart and Stephen's apology to his brother strike me as totally fake.
FYI, NYU's bariatric program requires a year's worth of psychological therapy and nutritional counseling before surgery. Why is therapy always a last resort on this show? Why are they handed a print out of a diet when most have no idea what a carb is? So much distracts me from the willing suspension of disbelief.
I was a fat teenager and have to work hard at diet & fitness so these shows keep me on the right path, much like A&E's Hoarders did for my apartment.

I watched one of these "600 Pound Life" shows last night. Its subject was a woman named Nicole, a blob with a chest tattoo and two piercings in her lower lip. Although she was the size of a hippo she nonetheless had a husband or boyfriend (an average looking guy with an average weight) and somehow they managed to have sex (he would have had to lift her belly up to get at her crotch) and she they had a couple of kids. It amazes me than there are always people, both male and female, who are willing to become involved with people who are morbidly obese; it would seem to me that the obesity would be a huge rad flag. Since his lady love was too big to fit in a bathroom, her long suffering hubby or boyfriend had to take her out in the back of their small dwelling and hose off her naked body like she was a circus beast. It was not to be believed. Anyway, these 600 Pound Life shows are the equivalent of the freak shows that sideshow carnivals used to do and which were eventually outlawed. It's the same thing: these people are put on display for entertainment. And they are indeed freaks.

R242, I'm a nurse but no expert here; nonetheless, I think these skin removal surgeries are more for function than form--to help mobility and decrease infection. They may be misshapen but with hard work and then surgery they've extended their lives for decades. If you've lost a large amount of weight and are over 30-35, the skin doesn't bounce back and even the greatest surgeon can't carve out a beautiful form. You end up looking lumpy.
I'm not a Dr. Now apologist. I think he's way too knife-happy and does not view his patients holistically. I'd never let anyone I care about go to him.

He doesn't oblige people with sympathy, which is an approach that, for myself, I like. And he does get people get the emotional/psychological help when the need it, so at least he recognizes some people need it. R224, he is a surgeon who is performing surgery on patients whom most other doctors wouldn't touch with a ten foot pole. He is not trained in plastics. He is trained in vascular surgery which is why he deals with lymphodema, a very complex disorder which is often non-operable. You can't just cut these huge masses off, you're lucky to have them reduced. He is not aiming for pretty, he is aiming for functional.
Even the best plastic surgeons can't do much for the formerly morbidly obese whose skin has lost its elasticity. Healing is difficult, especially if the patient has eaten his or her way to diabetes. Look at that hobgoblin, Mama June. She's had multiple surgeries but she'll never look proportional or "normal". The human body is miraculous but not supernatural. It wears out with abuse.

Amber Rachdi was/is a lovely young girl who had the worst lymphoedema i have ever seen on that show. She has achieved her goal weight, is happily married now and saving up for skin removal surgery. She is a breakout star though and unlike all of these much older, more solidified addicts, acknowledges food addiction as omething she must resign herself to forever. She says this surgery does nothing but buy you some time to rehab yourself. These people are at death's door. Having the surgery gives them a little time (before they go back to addictive over-eating to lethal levels), to get therapy and to come to grips with a horrible addiction.

In one season of Skin Tight there where two formerly obese persons ready to have their skin removed. One formerly obese person went to Dr. Now to get the skin removed and another one went to expensive plastic surgeons in Austin. Trust me, Dr. Nows patients looked so much better and several of the ones done in Austin had surgery complications and had to go to Dr. Now to get it fixed. By the end of the season it was obvious he did the better job both medically and aesthetically. There is only so much fat that can be removed and veins that can be reattached.

They did a follow up show this week on Lupe; her story stuck out in my mind because the first show that featured her showed her getting stuck on a toilet. She'd lost some weight and wanted to try and use the toilet like a normal person (her husband boyfriend Gilbert had to clean her up like a diaper-wearing baby when she relieved herself) and promptly got stuck on it. It took several men to extricate her. Anyway, she'd been doing well enough to have the weight loss surgery; after having it her husband Gilbert couldn't keep his hands off her still gelatinous body, so she had to go to the hospital to see if any of her stitches had popped. I didn't watch the show closely, but at the end of it she leaves the long suffering Gilbert (he's cheated on her and who could blame him?) and she thinks he's too demanding (he takes care of her like she's an infant and she thinks HE'S too demanding?) and goes to live with a cousin. Gilbert tells her if she wants to come back he'll be waiting for her. If he has any sense he'll find a normal sized woman and move on with his life, but maybe he really is that codependent.
Obviously these shows are scripted (ALL of the 600 pound blobs sound exactly alike when telling the story of their lives) and once you're seen one you've pretty much seen them all. Only a few seem exceptional, like the case of the Assanti brothers; I don't think I've seen anyone on these 600 pound life shows who equals Steven Assanti in terms of repulsiveness and psychotic behavior. I think some of it is acting but he really does come across like a soulless piece of shit. I wonder what will become of his brother, though. Maybe they'll devote a show to him.

Sure, the hubby had washed her ass and taken care of her, but like the other enablers we've seen on this show, he got off on that, because it allowed him to dominate her and remain in control. As soon as she started getting better, he started acting out--a pattern that's been shown on the show time and time again. Lots of marriages on this show have not survived the morbidly obese person's weight loss. When the fat person gets better, it interrupts the sick dynamic that the enabler wants.

I believe that part of the show was entirely made up by the writers. Gilbert was so hot for Lupe's elephant bod that he simply HAD to have sex with her right after the surgery? I remember some other show about a morbidly obese woman named Connie: "The Incredible Shrinking Woman." She was over 600 pounds and had weight loss surgery (she wasn't attended by Dr. Now); due to lack of circulation her inner thighs started to rot. It was beyond disgusting. She recovered from that and traveled to England (who was paying for all this? The show?) to talk to a surgeon about getting the skin under her arms tightened up and having her breasts done). The surgeon declined to do the surgery because he said it wouldn't be a good idea to do it so soon after her having such a severe infection. Mightily pissed off, she storms out of his office after saying "I guess we don't have anything else to talk about." Later she was shown eating and crying. She said that's what she does when she gets stressed or upset: she eats and cries. Of course she couldn't eat a great deal because the weight loss surgery had reduced her stomach to the size of a walnut, but she was reacting to stress the same way she did before she had the surgery. The doctor in England commented that you could surgically remove excess fat and reduce the size of the stomach but you couldn't change a person's mental state. In others words, you can take the fat out of the body but you can't take the fat out of the mind. Connie, "The Incredible Shrinking Woman", was the subject of two shows, but after the one where the doctor turned her down for surgery there weren't any more, maybe because she wasn't making any more progress. I think that same thing happened with "The Half Ton Teen", Billy Robbins. His story was a two parter also, but after the second one there weren't any more, probably because the fat fuck was still morbidly obese.

I wanted to add that thin people suffer from lymphedema too. It can develop after surgery and radiation treatment for cancer. I was very worried about lymphedema following my own treatment for breast cancer but fortunately I seem to have escaped it.

Sorry if that's a stupid question, but why aren't these guys put in a coma and then drained of all the fat, cut off all the extra skin and then kept in a drug induced coma for awhile until the body managed to adjust to the new body (IF they really want to lose all the weight) and then practice to eat healthy and work out regulary?
