I had acne as a teenager, but since my early 20s not really much of a problem. Recently I've had a few zits on my face, which is unusual, but they've been healing. But suddenly this evening I discovered I've got a boil on my shoulder. What could be happening? I've not changed my diet or my routine or anything.
Why would I suddenly have an acne outbreak at age 48?

Use sulfur soap. It will clear everything up - you only smell a little bit. And then you don't need it anymore.

Body chemistry changes with age, OP. I believe you when you say nothing has changed.
Although I would say if your routine has changed due to COVID there's a possibility that had an impact.
A boil can be stress related but is also bacterial in nature. You may want to consider using tea tree soap or Hibiclens to wash around your neck.
As for cleansers, I swear by this. I had a similar flare up about 5 years ago and started to use this and have had no problems since. You can use with a washcloth for a gentle cleanse, but maybe once a month get more of a coarse buff to do a bit of a scrub.

Oddly I also get boils on my shoulder come spring every year. Sometimes the change in light and weather can do a number on your skin hormonally. Just go to the dermatologist and see. It normally clears up in autumn for me and the more active I become.

I started having a lot of outbreaks esp my shaving area (my facial shaving area) and my chest around 50. My dermatologist prescribed some shampoos and cremes and antibiotics that didn't really work. I went for one more visit mostly to chew him out for wasting my time. However, that visit he had an intern with him, and when the older doc left the room he suggested Hibiclens (a heavy duty antimicrobial sold OTC). It worked completely within three weeks.

Get checked for diabetes. If your blood sugars run high, you can get infections easier. In the meantime try putting those hydrocolloid dots on your zits.

Acne can set in at any age. Although less common in adults it happens even with folks in their 70s and science was not yet able to find out why it may set in later in life. . Don’t waste your money with over the count products or miraculous recipes from friends. Just visit a good dermatologist and he will ask for the right tests. If your acne is severe and there’s no underlying condition you will probably get a prescription for Isotretinoin. After 3 months you will get your flawless skin back to where it was.

I have Rosacea but it is now under control. Adult Onset acne, cystic acne and Rosacea acne are not the same things.
