I visited a children's polio 'treatment' facility in the backwoods of India. The treatment involved stretching out the contracted limbs to their physical limit, then applying a plaster cast. Keep the cast on for a month or so- then re-stretch and re- apply the cast. Repeat until the limb returns to a somewhat normal state in terms of flexibility. The 'issue' is this stretching is incredibly painful for these young children.

Is this from a wild strain of polio or from the virus in the live oral vaccine most commonly administered in poor countries? The majority of new cases in recent years came from the latter. They need to switch to using the killed vaccine as no cases of polio ever arise from it.

In the UK, the polio vaccine is part of the NHS routine childhood vaccination schedule.

It's given as a jab when a child is 8, 12 and 16 weeks old. And two further shots are administered at 3 years and 4 months old, and at 14 years old.

However, one in ten kids in London aged five are not fully vaccinated against the bug.

Polio epidemics did not start until the end of the 19th century - but the illness was around long before that.

It ravaged children in Victorian Britain, causing them spinal injuries.

You do have a childhood vacination program. But again the main risk of spread in the modern western world is from immigrants from poorer countries who may not have been vaccinated. Apparently Pakistan and Afghanistan have an epidemic of it and the number of refugees fleeing the Taliban right now that doesn't bode well.

This parallels measles outbreaks in ultra orthodox communities in the US. Measles can be a deadly illness, mostly for under fives.

I was a Peace Corps volunteer in Nigeria in the 90s. In the North there was resistance to the polio vaccine and it was so sad to see the stricken children.

I want to add that in Nigeria I saw people with advanced leprosy. It was like being back in biblical times. Leprosy is easily cured with a combo of meds that cost no more than $1.

Antibiotic-Resistant Typhoid From Asia Is Spreading Worldwide

Treatment-resistant typhoid originating mostly from South Asia has springboarded across borders almost 200 times in the past three decades, according to new research that underscores the increasing global threat of infections that can evade antibiotics.

Between 2014 and 2019, scientists sequenced the genomes from 3,489 cases of S. Typhi, the bacteria that causes typhoid fever and kills over 100,000 people annually. Data from four high-burden nations -- Bangladesh, India, Nepal, and Pakistan -- were combined with an analysis of 4,169 similar samples from more than 70 countries over a 113-year period, making it the largest study of its kind.

The findings, published in The Lancet Microbe, showed that while resistance to first-line treatments generally declined across South Asia, global issues remained. The number of strains able to overpower macrolides and quinolones, two important types of antibiotics, climbed sharply and frequently spread to other countries, the study found.

For years scientists have beaten the drum about rising cases of deadly bugs that can survive treatment with the most potent antibiotics. Drug-resistant diseases killed more people than HIV or malaria in 2019, according to a separate study published in January. Recent examples include rising infections in the US, along with last year’s deadly fungus outbreak in India, where the misuse of antibiotics in humans and animals is exacerbated by poor sanitation.

The findings are “a real cause for concern,” said Jason Andrews, an associate professor at Stanford University and the study’s lead author, urging that prevention measures be put in place, particularly in high-risk nations.

“The fact resistant strains of S. Typhi have spread internationally so many times also underscores the need to view typhoid control, and antibiotic resistance more generally, as a global rather than local problem,” he said.

The study, funded by the Bill & Melinda Gates Foundation, had some limitations, including the underrepresentation of samples from endemic regions such as Sub-Saharan Africa and Oceania. The S. Thyphi genomes only covered a fraction of all typhoid fever cases, meaning the researchers’ estimates probably undershot the true scale of global spread and resistance.

The United States is among the more than 100 countries that does not meet recommended vaccine coverage thresholds to control a potential outbreak of polio, leaving America vulnerable to an outbreak of the devastating disease.

Only 46 nations who provide data to the WHO have met its recommended 95 percent target, including Australia, China and Japan.

But uptake is less than half the level it should be in some nations.

Just 41 percent of one-year-olds in Papua New Guinea are vaccinated against polio, with rates only slightly better in African nations Central African Republic (46 percent), Somalia (47 percent) and Guinea (50 percent).

Polio can be a tough disease to track, as a vast majority of cases will not display the devastating symptoms that have become synonymous with it.

Many infections are either mild or asymptomatic. Around one percent will result in paralysis and even death, though.

Children are believed to be most at risk from the disease, though it can afflict adults as well.

The United States has remained free of polio for nearly three decades. The last case detected on American soil was in 1993, when someone traveled into the country carrying the virus.

No cases have originated in the U.S. since 1979, marking 44 years since there has been an U.S.-borne infection.

I had a childhood series. As an adult, part of a batch of jabs for a trip to India, a "lifetime" polio shot was included as I recall.

R30 I worked in healthcare in Nigeria in the early 90s, when polio was endemic there. I was vaccinated and given boosters of just about everything, so no I didn't get polio. Malaria was a bigger problem for those of us not exposed at earlier ages.
