I think it was Cass.
For the longest time, Dean would obsess over Sam. He would do anything for Sam. He would be jealous if anyone else took Sam away from him.
But when Cass came into Dean's life, everything changed.
At first, Dean didn't know what to think about Cass.
Then he became friends with Cass.
And by season 12, he realized that he was in love with Cass.
It's a love story for the ages. DESTIEL was DESTINY.

Cass appeared to Dean in the motel room bathroom,then took him to a whore house.
That's when Cass realized that he didn't like women, but instead was attracted to Dean.
