Have you ever bought a tailored shirt, pant, or suit from an online custom tailor?
There are a number of online tailoring businesses that will make you custom clothing if you either send in your measurements or mail in a shirt/pant you want to copy. An example is Modern Tailor.
There are even a few app-based tailors that figure out your measurements with body photos.
Have you ever bought from them?

I am hard to fit and a piece of clothing either feels right on me or it doesn't. I don't buy online for such items, but I have seen some ads for this type of fitting and it looks convincing.

Best thing to do is ask around your town at dry cleaners or menswear stores and they may put you in touch with a little old Lebanese or Italian or Chinese man who learned his trade in the 1960s and can make you your shirts. He will have good quality cotton shirting fabric but you will generally need to source and find better buttons than he stocks.
