My coworker had a similar shaking episode. She has the same build as Merkel, and she has diabetes. She says she gets the spasms after sitting for long periods, and also after getting out of bed in the morning. We were very alarmed by it, but she just continued on with what she was doing like nothing happened, as she is so used to it now. Its probably a side-effect from diabetes and medications for it.

I can only imagine the stress levels some of these individuals endure.
Over time and with augmented behaviours, this can impact corporeality. (Looking at you👀 Trump.)
Can think of a few possibilities outside of stress or anxiety. As some have mentioned, neurological problems or autoimmune and/or metabolic disorders.

r63 Trump doesn't experience stress because he doesn't care about anything other than how he's portrayed in the media. And even then, it's more anger than stress. That's also why he seemingly hasn't aged at all since 2016, which is what happens to most US presidents who actually WORK, not golf all the time.

Anger does impact the human body.
(See studies relating the emotion to cardiovascular events and higher incidences of cancer....to name just a couple.)
But with Trump, it's anybody's guess as to what's going on.
Personally, I believe dementia or some type of mental impairment seem to be revealing itself at the moment. His speech patterns and body language could be early FTD.

What's striking is that her facial appearance seems fine. She's not flushed or pale, just looks worried. If it was blood sugars or thyroid you'd expect sweats, dizziness, something like that.
And if she can keep traveling then it's apparently not incredibly serious. Baffling.

I had a close friend who died of Parkinson's Even near the end, there was little shaking. Is that because of medication? I think so, but do not remember.. I do know the disease got progressively worse - but slowly. To the degree, he ended up in nursing home, because his family could no longer care for him.
If Merkel does have Parkinson's, it should not effect her remaining time as Chancellor.
