I don't know if this is the sort of thing you're referring to OP, but I remember a while back a DLer warned about people, especially men, who wear a ton of rings. Also from Chinese Medicine, the tongue gives away a lot about a person. A "heart crack" is when a middle crack in the tongue goes all the way to the tip, and a pointed tip, are both signs of being mentally unbalanced. A red tip or lack of coat indicates propensity to rage or fly off the handle.
A thick-coated or greasy-coated tongue can also indicate something is off mentally, though can be more short-term issues. Think Miley Cyrus when she was acting out, and now when she sticks her tongue out it looks more normal and she is not as out there as she was.

I offered a nutritionist a cookie at a social event: one sugar cookie. Runners often have crazy eyes. I've always felt they use running (consciously or unconsciously) to control their mental health.
I watched the eyes of a loved one transform as he sunk into a deep depression and eventually killed himself.

Going back to the eyes there are few things. Eyes darting around, perhaps frenetically or just unable to fix on one point. Especially when coupled with a tense stance. It means they feel unsafe, and in perfectly normal, safe situations it looks out of place. Feeling unsafe is also associated with avoiding eye contact, in Western culture. Again in particular when their body is tense.
Eyes that have dulled, is associated to depression. The life draining from their eyes. In opposition they will have limited movement.

Sometimes dilated pupils are a sign of elevated stress-hormone levels. Normal, non-disordered people in long-term abusive relationships have high stress-hormone levels, specifically elevated adrenaline and cortisol levels, which can cause the pupils to be dilated. So, sometimes dilated pupils are because the person is the abuser, the crazy one OR it is because they are being abused.

As far as the weight comments here, I agree. But wanted to add, I've noticed it is extremes; extremely underweight or extremely overweight.
So someone who is 10 or so pounds underweight, or someone 20 or 30 pounds overweight, probably isn't crazy unless they also have most of the top seven signs of a psychological disorder. The key is *extremes.* But that doesn't mean people who are a normal weight, or not too far from it, are normal. It depends if they also have most (or all) of the top seven signs of a disorder.
I've also noticed their facial muscles tend to look tight or tighter than usual for no apparent reason. Their smiles are forced or fake as is their laughter. They may also smile and/or laugh at the most inappropriate time. Quite often, (although not always) their body language does not match their words, sort of like a disconnect between the two. That blank, haunting, longer than is customary stare down some of them give off is also a dead-giveaway as are facial/neck tats, multiple piercings, and obsessed with pit bulls.
Some of them are pleasant and charming and appearance-wise look perfectly normal and they know how to work you by saying what you want to hear. So it's not always easy to determine crazy by appearances alone, or appearances and initial impression, without really knowing the person, but it can be done. But since one-third of the population is crazy it's a skill everyone should learn and continuously develop.

I wouldn't count obesity as a signifier, unless you want to categorize most modern Americans as crazy. Most of the time, a fat person is just a fat person.
I've learned to recognize bipolar/psychotic types of crazy in peoples' eyes and facial expressions. It's a kind of tension/panic/non-rational lizard brain microexpression that lingers longer than it would in well-medicated/healthy person's face.
For non-physical manifestations, I agree, the lack of long-term close friends and especially, multiple fallings out, and not speaking to former friends and family members is a red flag for personality disorders like BPD and NPD.
For sociopath narcissists, I notice they'll be described by widely disparate acquaintances with over-the-top complimentary words that sound like a planted script of superlatives. The sociopathic narcissist plants that script, mirroring and putting on an act, all things to all people, and the most wonderful creature ever.

People with really sloppy, cluttered, dirty cars and the same for their houses. I don't mean their homes are a little dusty here and there and not very neat and tidy because they aren't good housekeepers. I mean sloppy, dirty, thick layers of dust, dirty floors, overstuffed closets, and crap that should have been thrown out long ago taking up space and clutter, clutter everywhere but not necessarily hoarding. Also, hoarders too. I've never met a hoarder who is not a mental case.

While I agree that obesity alone might not be a sugn, if you watch some of the people on My 600 lb Life for example, a good few of them are master manipulators if not just batshit crazy.
They use their weight to force their families to take care if them, even their kids. It's sickening.

Barring medical conditions, extremely underweight OR extremely overweight, as in more than 20 pounds under, or 80 pounds over. That would be extreme. I've never known a person to be "extreme' one way or the other, and not be mentally screwed up. Except for someone I knew and loved dearly, who was a great person and was 25 lbs underweight when he died of AIDS. Most people with extreme weight issues are nuts.

Another sign I've noticed about "crazy" is that quite often their body language and words do not match. I have noticed that more in the workplace during typical daily business interactions than anywhere else. But I've also observed this with a few of my crazy relatives.
