When I have sex (which is almost never), it's bareback. I can't get hard with a condom in sight. I've had enough limp dicks that I refuse to even try anymore. From about 92 to the early 2000s, even beating off was a problem because I couldn't get off to the condom porn and even the precondom classics got old.

Throughout the 80's I used to bareback only after getting test with committed monogamous long term boyfriend.

But around the mid 90's i noticed a general behavioral change with my hook ups. I'm a big dick top and when I was single, even before PreP was around, 95% of my hookups wanted me to fuck them bareback. all assumed i was neg without even asking me what my status was, just because i'm a somewhat handsome guy, have a big dick and have a good body, so of course i was "clean". This was through the 90's till early 2010's again this was before PreP.

Now i'm on PreP still a top but i've been experimenting with being a bottom. So far I've let 5 to 7 guys (HIV statuses unknown) fuck me bareback. My PreP protocol has me visiting my doctor's office every three months for an HIV test and full STD panel and as of today, I'm still negative. Judge all you want, but I'm confident using PreP.

I wish more guys would get on PreP, not just to bareback, but it forces you to get tested every 3 months, so if you do pick up some STD or HIV,, you'd know pretty quick. And if you do become HIV+ the meds will render you for the most part undetectable and thus unable to spread the disease to others.

HIV's biggest problem is that there's guys running around saying they're neg because they got tested 1,2,3 years ago and don't know their current status, so they keep assuming their neg and maybe they're not and unfortunately spreading HIV around. At the very least if you're on Grindr or fuck around with strangers, just get tested every 3 months. At least until there's a cure for HIV, regular testing is the only way we're going to stop this disease.

Even if I were on PrEP or knew for sure my hookup was on it, I'd still be too terrified to bareback. There has been anecdotal evidence of transmission on PrEP, and that's enough for me to be paranoid and say no thank you. I still bottom, but only if the top wears a condom. Honestly, it's gotten more and more difficult to find anyone else who insists on condoms.

You "resistance" people need to knock it off. The cocktail is made from nucleoside/tide and nonnucleoside reverse transcriptase inhibitors and protease inhibitors. These are classes of drugs that pretty much all work the same way regardless of the name. They've been in use since the late 90s.

20 years has elapsed and the drugs are still working keeping people alive. For you "resistance" people to even be remotely right, HIV would have mutated and people would be dying right now. The experimental evidence says otherwise. You need to do nothing more than research resistance to antibiotics. Those drugs were losing effectiveness within 3 years of their development. The mechanisms are just not the same.

They still use AZT (zidovudine (Retrovir)) even though it's called by a new name, but it's still the same old drug that people said would make HIV mutate.

I have read many, many stories about people who feel they may have had it for 5, 8 years without realizing it. I have spent countless hours on POZ forums, even though I'm negative. Call it a sick pass time. The good news is, if you test negative now, you know that you're in the clear and don't have to go through the weeks and months of waiting for the confirmation test.

r56 No. In the early days of the drugs when the viral loads went to zero, they took people off the drugs to see if they were cured. When it was determined that there was a reservoir that would kick into production again, they put them back on the drugs, and the viral load went back to zero. That's why they started to say "undetectable." There's always a tiny amount of virus, and for the "resistance" theory to be correct, it would have already happened.

Stop confusing resistance to antibiotics with antiretrovirals. They are NOT the same.

The meds don't stop working. It's your body's ability to handle the side effects or metabolize the drugs that stops working.

Look, back in 2000, I would have been with you, but as the years ticked by, it became obvious that the tiny amount of HIV in each compliant person should have mutated. My friend's on the same drugs today that he was taking on 2005 along with the meth addition that he developed, and I know he's not 100% compliant, but he still has undetectable viral load tests when he gets back on his meds. I doubt he's the only one.

There's all those poor, uneducated people in Africa that are probably getting just the bare minimum of a regime that works, and yet, the drugs are still working. That's a population that's guaranteed not to be compliant.

I was thinking the same thing, they want it to fail, against all scientific studies. PreP has proven to be more effective than condom use, google it. But then the Debbie Downers say, "what about other STDs?"

Well unless you put a condom on every dick you suck or touch, you are vulnerable to Chlamydia., Gonorrhea.,Syphilis., Herpes among other things. Just cuz your fucking with a condom does now magically protect you from other STDs. Boys your throat and hand is an entry point for all the above.

I believe this is setting the stage for a super-AIDS. We're essentially back in the 1970s when everyone was promiscuous and fucking anything and everything raw, and at the clinic for a shot of penicillin every other week. I am bisexual and have never done it bareback/raw with any of the women or men I have had sex with. I just use condoms and have always used them, and assume that whoever I am having sex with could be HIV+. Nobody has answered a question that was asked here: when is it safe to go off Prep? As someone that's HIV neg and uses condoms, it seems to me that most of the men on PREP were into barebacking long before PREP/Truvada was marketed as pre or even post exposure medication for HIV, and that some have the mentality that with the high risk sex they have they'll eventually become HIV+ or have or will get other STDs/STIs so why not just start taking the meds?

Condoms are cheaper, don't have horrible side effects, and when I have anal sex as a top only I use them. I talked to my doctor and he knows that I am mainly into oral sex and he said how there's no need for me to go on PreP since I do low risk sex acts.

R269 it's not being puritanical and "sex-negative". I've had friends become HIV+, who lived with AIDS when their medications stopped working or they were not on them, and who have other STIs including hepatitis C, herpes, and HPV.

I'm no angel but if you use common sense, use condoms, and have safer sex it's easy to avoid becoming HIV+ and getting other STIs. I have heard from medical professionals that nowadays it's easier to avoid becoming HIV+ if you use condoms, but how if you don't use them and even if you are on PREP/Truvada that the medication may not always work or keep you HIV neg and that you run the risk of getting other nasty STIs and bacterial infections.

Hi, R303. Yes, I have Lyme. I was first diagnosed 20 years ago. I am doing a lot better right now and I’m off antibiotics at this time, but the past decade of my life was a living hell that seems surreal in retrospect. Now life is basically chronic fatigue with lots of random short circuits, but for years it was that plus indescribable pain, transient paralysis and unbearable anxiety with obsessive visions of myself committing suicide. Also cluster headaches. Google cluster headaches. Neurologist speculate that the pain of cluster headaches may be the most extreme pain known to humankind. Women who have them report that the pain is far worse than the pain of childbirth. A side effect of my Lyme infection is mast cell activation disorder. I’m allergic to all kinds of things including—no one will believe this but it is absolutely real—heat and exercise. I break out in rashes and become disoriented with double vision and sometimes anaphylaxis when I overheat. So being a typical gay gymrat is something of a bygone era for me. I used to work out 4-6 days a week and was an avid cyclist, riding 35-40 miles on most weekends when I was young, which is most likely where I was bitten.

For those who think the best course of action is to stick a raw dick up your ass...enjoy your wellness while you can.

I feel for you R292 / R304. I lived a very similar story. I'm doing well, at the moment. I push myself every day. I've actually joined a gym and am taking it more seriously than I ever have, I'm doing it as if my life depends on it, which I really think it does. Oraquick has a HUGE false-positive rate. Let's suppose you hook up with some hot guy on Scruff, both do Oraquick tests, and his says "positive". Are you REALLY ready to deal with a relative stranger who's just gotten news he probably wasn't in any mental state to handle? Or for the roughly 1-in-20 test where YOU test poz? I guarantee, even if you get two ELISA tests from your doctor, he does a Western Blot just to be sure, and they all say you're negative... you're going to be fucked up and neurotic about it for MONTHS.

If you REALLY want to be fair about it (and save money... Oraquick isn't cheap), run a single test with spit from BOTH of you. At least then, you'll both be equally mindfucked if it comes out poz, since it could plausibly be either one of you.

Or... do PrEP. If the other guy is poz, you'll still be almost as protected from HIV as if you used a condom. If the other guy is +/u AND you're on PrEP, your likelihood of catching HIV is roughly equal to your likelihood of dying in a plane crash where you WEREN'T a passenger.

Basically, avoid having sex with guys whom you have reason to believe lack good health insurance... and guys who insist they're "clean" and act insulted when you notice that their last test was 3 years ago (they're the ones most likely to have a raging, uncontrolled HIV infection with viral load in the millions).

Guys look at the studies, PreP is safer than condoms. I know its hard to believe but almost every major health organization has concluded that it is safer.

AND a person that is undetectable he CAN NOT INFECT YOU WITH THE HIV VIRUS. Again google the studies. these are major studies that are trying to save lives. And for those lucky enough to have lived through it all and now are on the right medication and are "undetectable" they CANNOT transfer the virus! The CDC just finally release a statement confirming that.

Plus, if condoms where so effective, HIV transmission would have tapped out in the 90's but it didn't. why didn't it? most guys in the real world aren't using condoms. so what I'm saying here is, yes use condoms if that's what you personally feel is right for you. but for those of us that don't like condoms or like cum up our asses, we now have PreP.

A few other observations about HIV:

* Not all strains of HIV are equally likely to successfully infect a new host.

* In most cases, the mutations that confer the greatest resistance to the drugs in Truvada ALSO seem to impair the strain's ability to infect a new host (particularly, its ability to establish a viral reservoir).

* Genome analysis of recently-infected individuals seems to indicate that a relatively small number of strains account for the overwhelming majority of new HIV infections... and most of those strains are STILL very susceptible to the drugs in Truvada.

In other words, Peter Poz might have 47 strains, 46 of which are highly resistant to Truvada & account for 99% of the HIV in his blood & semen, but if he infects Bob Barebacker and Bob's HIV gets discovered a few weeks later and has its genome analyzed, most of the HIV in Bob will be from strain #47 -- the one that's NOT resistant to Truvada.

I forgot to add... the difficulty some mutated strains (esp. those that emerge late in an uncontrolled HIV infection) have establishing a persistent viral reservoir means that if the patient DOES eventually manage to achieve undetectability through more exotic multi-drug cocktails, many of those drug-resistant mutated strains will ultimately be gone completely once cleared successfully (because they never established viral reservoirs of their own).

Basically, undetectability doesn't just STOP the mutation clock... it can sometimes partially RESET it, too.

In other words, the strain that resulted in someone's initial infection will be with them forever (because it will re-establish itself from its viral reservoir if meds are discontinued), but multidrug-resistant strains that were present at some point in the past might NOT persist once they're finally cleared unless they established reservoirs of their own.

This implies that someone with advanced AIDS might need several additional meds to BECOME undetectable, but doesn't necessarily have to continue taking all of them forever to REMAIN undetectable. Once undetectability is achieved & maintained for a "long time", the additional meds can be withdrawn one by one (with frequent monitoring to quickly discover if one of the resistant strains DID establish a reservoir and rebounds without the necessary drug to control it).

Actually, we DO. Lamivudine (a/k/a "3TC") has been around for ~25 years, and it (or its close chemical cousin emtricitabine ("FTC")) is found in nearly every combo med INCLUDING Truvada. Tenofovir (the other drug in Truvada) has been around for almost 20. Among other things, both drugs have been routinely given to negative-testing infants with HIV+ mothers for at least 15 years, with one of the tamest safety & side-effect profiles of any drug in modern history. Compared to Truvada, ibuprofen is risky, and Tylenol is downright LETHAL.

And in any case, what defines "powerful"? Take 10x the normal dose of Tylenol, and you'll probably die from irreversible organ failure a few months later. Take 20x the usual dose of ibuprofen, and you'll be in agony for months with bleeding ulcers Take 90x the normal dose of Truvada, and the only real consequence is the waste of a drug with a retail price approaching $10,000.

R354: it's not inconceivable that the meds purchased online are fake, but it's probably less likely than you'd imagine... ESPECIALLY if you buy it from someone in India.

In the US, you can patent a drug, a dosage regimen, AND a manufacturing process. Indian IP law recognizes ONLY patents on manufacturing processes. So in India, Cipla manufactures generic Truvada with complete legality, and India's equivalent of the FDA is generally recognized as being competent and effective at regulating generic drugs (partly because generic drugs are a MAJOR export industry for India).

Generic Truvada isn't all that expensive to manufacture. 99% of Truvada's cost in the US is due to monopoly profits due to Gilead's patents. In India, a 30-day supply of generic Truvada (Cipla's "Tenvir-EM") has a RETAIL PRICE of approximately $35 (compared to the $1,800+ retail price for Truvada in the US). At prices that low, it's barely even WORTH trying to make genuine-looking fake generic Truvada. Learn what Tenvir-EM looks like, and if the tablets you got look like that, they're probably genuine.

Things are going to get interesting in the US early next year. The last "blocking" patents on tenofovir are due to expire any day now, and Cipla ALREADY has an FDA-approved generic similar to Truvada (Tenvir-L... like Truvada, but with lamivudine instead of emtricitabine) that it could conceivably sell to US pharmacies with complete legality starting early next year (lamivudine and emtricitabine both ultimately have the same active metabolites... emtricitabine is more potent, so you can take less and end up with the same amount of metabolite, but on the flip side, you can get exactly the same effect with lamivudine simply by taking more). Technically, Tenvir-L hasn't been approved for PrEP... but in the US, doctors can prescribe ANY drug that's approved for something for any other condition they deem appropriate, so Tenvir-L's approval as a HIV treatment is more than sufficient.

It's unlikely anyone with health insurance will bother, but I can easily see doctors with uninsured patients writing them prescriptions for Tenvir-L, which would probably end up costing around $100/month by the time chains like CVS and Walgreens get their hands on it. $100/month is expensive, but it's at least within the realm of sane out-of-pocket affordability... $1,800/month just isn't happening unless you're literally a millionaire.

R372: The fundamental problem with "on-demand" PrEP is the fact that PrEP doesn't prevent HIV from infecting cells, it works by preventing it from REPLICATING so it can't infect even more. Infected T cells might take a few weeks to completely die off. If you discontinue PrEP before the last infected T-cell dies off... you're quite possibly going to get infected "for real".

Taking Truvada for a few days after exposure might still provide ~80-90% as much protection of taking it daily, but the ongoing "seesaw" effect of Truvada-noTruvada puts you at significant risk of developing FTC resistance (which is why guys on PrEP are supposed to get HIV tests every 3 months... if you were taking ONLY FTC or 3TC and became infected, resistance could develop in 2-8 weeks if you weren't even taking THAT consistently. Add tenofovir, and the window increases to ~3 months w/irregular dosage and 6-12 months with daily dosage. So 3-month retests catch the worst-case scenarios before they develop resistance.

The only scenario where "on demand" PrEP is safe:

1. Unsafe sex occurs

2. Double-dose of Truvada IMMEDIATELY afterwards (or ideally, at least a day or two before exposure.

3. Full round of HIV tests the following business day, with blood sample retained for further analysis if seroconversion occurs within 6 months

4. Daily adherence for a MINIMUM of one month... three, if first dose occurred more than 6 hours post-exposure.

5. Follow-up HIV tests when discontinuing PrEP and 12 weeks later.

In most cases, you'd be advised to switch to a 3-drug regimen if you took Truvada #1 more than 3 hours after exposure, just to swat down any extra HIV a little harder. In other words, PEP, not PrEP. ESPECIALLY if you had reason to doubt whether the guy who fucked you was on PrEP or undetectable.

Due to the hassle & expense of all the tests you'd need after every episodic use, it's cheaper & safer to just keep taking PrEP daily (or to BEGIN PrEP as PEP, then transition to Truvada alone after a few months once infection is determined to be unlikely).

Just to repeat, though: the majority of resistance-risk falls squarely upon the person who uses PrEP inconsistently. Mutated HIV with resistance to Truvada tends to have impaired ability to spread. It happens, but it's extraordinarily rare.

Almost forgot... the reason for the additional retained blood sample in point 3 is to shorten the detection window slightly.

The problem is, no one test is perfect. There are highly-sensitive ELISA tests that can potentially detect a new HIV infection as little as 5-10 days after exposure... but those particular tests ALSO will react to common things that AREN'T HIV, and can't distinguish them from HIV antibodies. One specific example is rabies vaccine... the antibodies produced by rabies PEP are indistinguishable (to the early-detection tests) from the antibodies produced by HIV exposure. In fact, there was a period of a few years when some researchers even theorized HIV was somehow related to rabies, because someone noticed that veterinarians and zookeepers were frequently testing positive, despite never developing symptoms of HIV (and eventually, getting conclusively-negative Western Blot results).

Newer ELISA tests were developed to reduce the number of false positives, and these went on to become the ones routinely used... but THEY generally won't detect a new infection until 3-8 weeks post-exposure. Western Blot tests rule out pretty much everything besides HIV, and rarely fail to detect a real HIV infection, but they only work reliably at least 3-6 months after exposure.

So... you can see the problem. Someone thinks they were exposed to HIV. Their doctor does a broad-spectrum early-detection test, and tells him he's positive... maybe. If an immediate Western Blot test confirms it, he's definitely poz... but if it gives results that are negative, inconclusive or anomalous (likely), the doctor will tell the guy to come back in 4 weeks. 4 weeks later, the doctor does a less-sensitive ELISA test... it comes up poz, so the doctor does another Western Blot. This time, instead of showing negative, the results are "inconclusive". At this point, the guy is told he's "probably" poz, but there's a small chance he's not... but they'll have to test him again in 4-8 weeks, and won't be able to REALLY say "for sure" until six months after exposure (and THAT assumes the guy has been celibate since exposure. Otherwise, a negative WB test at 6 months just proves that he wasn't infected half a year ago).

Fast forward to the 21st century. We can now do genome-sequencing of blood samples and test for HIV directly (instead of depending upon the presence of antibodies). In theory, if someone had a viral load of zero a week or two post-exposure, they're almost certainly negative. The problem is, NOBODY -- not even people who've never been exposed to HIV in their lives -- will get a literal "0" result, because the test suffers from what can be described as "background noise". Influenza is indistinguishable from almost-but-not-quite-undetectable HIV. Ditto, for herpes, hepatitis, and plenty of other viral infections. So... what doctors will do is analyze TWO blood samples taken a few weeks apart. If the viral load indicated for the second sample is the same or less than the first sample, the individual is "probably" not infected with HIV. If the second sample's viral load is exponentially higher than the first sample, the individual probably IS infected by HIV. And if the second sample is higher -- but not exponentially higher -- than the first, well... come back for another test in a few weeks, because sample #2 might just be the result of some other viral infection.

I’ve been with my BF for over 10 years (mid-30s now) and we don’t use condoms. I would never have BB’d with other guys I dated because I never had this level of trust with them.

My opinion is that if you’re not in a monogamous relationship, you should use condoms. You really have to be honest with yourself, and if you know you’re going to BB, PrEP is the next best option. Like others have posted, get very frequent std checks if not consistently using condoms.

It’s human nature to be sexual, we should just be as responsible as we can about it. The level of risk/responsibility we take is gong to differ among individuals so it’s important to know which options are available to keep us as safe as possible.

Promiscuity has always carried health risks. HIV was the first risk factor that could actually kill you (well, syphilis could too, if not treated).

Prep doesn't eliminate the risks. Don't think a drug is going to save you. It can lower your risk, but you are capable of lowering your own risks more effectively (and without side effects).

For those who are in great fear of hep C, it’s curable now with a very well tolerated medication. I think the first one (Harvoni) is made by GSK, but Gilead - the folks who brought us truvada- has a newer version about to be marketed (or was just recently made available).

There’s still herpes and soon to arrive resistant gonorrhea (not to mention increased risk of anal cancer) to worry about....

I've thru this entire thread, and IMHO HIV rates are going up especially among the younger gay population. it is a fact that they're not using condoms, so in light of that, PreP is a good alternative. We need to lower HIV infection rates and if this tool reduces it why the hell shouldn't we all be advocating its use.

More likely is a profound decrease in HIV infection rates among middle-aged white gay men, with simultaneous explosion of new HIV infections in young white men.

Look at the explosion in teen pregnancies that occurred in the 70s and 80s. It happened because teen girls increasingly BEHAVED like they were "on the pill"... but most of them WEREN'T. Teen pregnancies didn't start to decrease until women started putting their daughters on birth control the moment they had their first period. The same will happen with PrEP. Until parents actively put gay tees on PrEP, a single unknowingiy-poz teen can infect half the sexually-active gay teens within 10 miles before anyone notices.

I respectfully disagree. They DO give a fuck about their health, that's why they're on PreP to begin with. because they know they are not going to use condoms, so instead of having completely unprotected sex without any sort of prophylaxis, they're on PreP which no matter what you think (Google the numerous studies) does give them protection against HIV.

Like I said before, no matter how many guys on here claim they're using condoms, they are in the minority. I'm a top and for many years now the guys I'm hooking up with simply don't bring the subject of condoms up anymore. For better or worst for many guys, condoms are not part of their sex lives.

So if PreP can prevent X number of infections, then let's get these guys on Prep. This idea that everyone on Prep is getting a sexual health check every three months is ridiculous. The majority of people aren't out to their doctor and don't want their sexuality on their medical record for employers to see.

Most of the guys I know on Prep get it online and don't ever have health checks. It's cheaper online but who knows what's in it. A lot of them only take it either side of the weekend to cut down on cost.

It's just a nightmare waiting to happen, and thank fuck it won't be happening to me.

R574: 3-month checkups matter less than consistent, daily dosing after the first year or so. If you're taking Truvada daily, your likelihood of being diagnosed with HIV after the first year is so remote, it's basically a total freak accident if it happens.

The thing is, this only applies if the individual really, truly DOES take Truvada daiiy without fail (no "disco dosing").

The fact is, if you somehow became infected (as in, established a persistent viral reservoir), you might not even seroconvert as long as you continued taking it religiously. This is seen ALL THE TIME in newborns with HIV+ mothers... they'll be put on meds at birth, discontinued after a year or two, then roughly 1/3 seroconvert within a few weeks anyway. Basically, the ones who seroconvert were "born poz", but the meds suppressed it well enough to keep them from seroconverting. What *nobody* agrees about is the percentage of the 2/3 who DON'T seroconvert who WOULD have seroconverted without meds during their first 2 years... it's believed to be low, but it's one if those things that are ethically-difficult to actually test since modern HIV meds are generally free of meaningful side effects & it MIGHT spare at least SOME kids from catching it "for real".

R581, I'm doubtful of those claims. Every body is different. Doesn't this depend on the age that you were infected? A child born with HIV will live until 80? A person having unprotected sex for decades will live that long? They're asking for other infections or cancer. Being 60 and poz seems like a miserable existence. Your fucking days will be over and because you led such an antisocial love life, no one will want you, as if you'd know how to connect to anyone anyway. You will look ill by that point.

Very true R575, all the younger and older guys who don't use condoms and believe that PREP will protect them from HIV and other nasty STIs are in for a big surprise when they become HIV+ or get Hep C, or maybe they know this will happen since they bareback (have unprotected anal sex), so they get started on the meds early?

The Truvada troll is lying, it's literally impossible to get a STD including chlamydia from mutual j/o.
