Everyone knows that r7, but even in the UK, people are aware of the other meaning. Perhaps the American usage is catching on there. Also, why was it "R-fag?" Maybe that was to distinguish it from the cigarette meaning.

If they called him "R-fag" -- a play on his tabloid nickname, R-patz -- then they were definitely using "fag" in the gay sense of the term, not the cigarette sense of the term.
