When did 'self-centered' become 'narcissistic'?

I was just reading 'self-centered' as a description of Philip Roth from a 1996 NYT article and thought quietly to myself 'Now they'd describe him as a 'narcissist'.

The word 'narcissist' is used constantly on DL.

I guess narcissist is a stronger word and therefore a stronger insult.

Or have I got it totally wrong? If so, please set me straight.
