Lots of reasons. Could be genetic, cholesterol, blocked arteries, stress.

You must be mentally slow, then.
Physical health can stave off the risk of heart disease but cannot prevent it entirely.

I saw him many years ago as the Engineer in "Miss Saigon" at the Masonic Temple Auditorium in Detroit and I remember thinking, well, Jonathan Pryce may have created this role but it's hard to imagine him being better than this guy. And maybe he wasn't, who knows. Kevin Gray was so vivid, so much fun to watch, and made you feel all the "ick" you should feel with the Engineer.
