There are different names for this but basically it's very intense itching of the scrotum, often in the middle of the night.

Perhaps you have a jock itch or a yeast infection, which is basically the same thing.
Buy a tube of Monostat that women use for their vajayjays. Actually, the more effective treatment is just like Monostat, but it's called Clotrimazole. They're sold right alonside each other. Both treat yeast infections; it's just that Clotrimazole may be more effective.

OP, I know you're not going to believe me, but I suffer from the same problem, and here's what I do:
1: I shave my balls and any hair on my penis shaft. Sweat is held by the pubic hair and allows fungus to ferment.
2: I have a handheld shower attachment in my bath. When I shower, I use the handheld spray and apply the hottest water possible (short of scalding water) to my balls and scrotum. This causes an incredible itching sensation, almost sensual in its intensity, and eventually I have to stop and itch my balls. I do that as long as possible. My theory is that this causes the liquid in pores that is responsible for the itching to empty. You may have to start the water at a mild/hot temperature and slowly raise the temperature to as hot as you can stand it.
3: After I dry off from my shower, I rub baby oil all over my balls. This creates a barrier to moisture and prevents the fungus from returning.
At the beginning, you may have to skip step 3 and just apply a good anti-fungal lotion. Do this for a few days until you're sure the infection is removed. You'll still experience the itching, but that's from the irritation caused by the fungus. Just do steps 1 - 3 and eventually, you'll be back to normal.

It sounds like a fungal infection. There's a solution, but it hurts like a motherfucker. Get some clove oil from a health food store. Mix it with water in a spray bottle. The stronger the solution, the faster it kills the infection, but the more it hurts.

OP...This is a classic case of why people are advised to get a second and third opinion. If this is as bothersome as you say, then it's some sort of bacterial, fungus, yeast or body chemistry dysfunction.
R11's remedy, while helpful, of applying baby oil is only temporary and is not a cure. Get a second or third opinion.
Separate issue, but for a couple of years, I had a doctor who burnt off a plantar's wart from the bottom of my foot. This was not a cure and only temprorary relief. But the wart kept returning. Finally, I got a second opinion from another dermatologist who prescribed 'Efudex,' which is a cream for skin cancer. Within no time at all, my plantar's wart disappeared. So two years of burning the wart did nothing. But the cream--from another doctor--disolved this deep, bothersome wart in about two months.

After several years of torment, I decided to try Scalpicin on my scrotum. It has really helped alot! I also only hand wash my crotch with a very mild baby soap. No wash cloth, no anti deorderant soap. All that was too drying.
I shower, hand wash my stuff, do a complete rinse, towel down, squirt about a teaspoon of the Scalpicin on my fingers and rub it in from back of scrotum up to base of my penis. I don't have the midday need to dig a hole in myself, I'm sleeping through the night. And many of the larger retail stores have their own brand which is a little cheaper. Be sure to compare the ingredients.

Try Lotrimin, which is made for Jock Itch. It also works great on Athlete's Foot and makes your toenails look fresh and clean. Chain drug stores like CVS, Rite Aid and Duane Reade all make generic versions which are cheaper and just as effective.
It comes in a spray but I prefer the cream in a tube.
I have it, too, OP. For me it's a patch of vitiligo (loss of pigment) that showed up in my twenties. Goes through cycles but itches like a MF, I've made myself bleed scratching it.
Posters above are determined that it has to be fungal, or bacterial, or viral, and if your doctors have examined you and said not, it's not. It's a non-specific dermatitis of unknown origin, which is a common occurrence.
One or twice a week when it flairs I put OTC 1% hydrocortisone cream on it. Takes care of the itching for a few days, but it won't go away permanently.
Quite frankly OP it sounds like eczema to me. When you have an outbreak of eczema the affected skin becomes very dry and a red patch will appear that causes intense itching. Scratching that itch usually ends up getting the area very raw which only increases the itch.
The best treatment for eczema is Benadryl (yes the same pill you take for allergies) and a good quality anti-itch ointment. The itch is happening because the body is producing an over abundance of histamines in the affected area. The Benadryl will reduce the histamines and the ointment will sooth the intense itching.
I imagine the reason it's happening mainly at night is because heat causes the affects of eczema to intensify, and your body produces a lot of heat underneath the covers at night. One good way to immediately reduce the itching is to take a wet bath towel and put it in the freezer for a few minutes until it's very cold. Then fold it up and make a compress for your boys (down there). Trust me, that will stop the itching immediately while you wait for the Benadryl and ointment to start working. Just don't take the Benadryl during the daytime because it's going to put you to sleep.
Hydrocortisone cream (generic),won't cure it but it will stop the itch.

Please try apple cider vinegar. fill a spray bottle and apply before (wait 10-15 minutes while you shave; apply afterwards and let dry either natually or with hair dryer(this is actually best) when dry there will be NO vinegar odor. Vinegar will kill fungus and other microbes(eczema is a micobial condition). If it's too strong dilute with distilled water. It may tak a few days up to a week to see results. Also, hormonal fluctuations can cause ball/crotch itch. Try the vinegar. This works and it's cheaper than pussy cream!
OP, make sure your Dr has eliminated the possiblity of scabies.

I've had it from about 11. Nothing suggested or prescribed ever helped. Doctors didn't care once they made sure I didn't have scabies or jock rot.

Listen Carefuly - I had the exact same problem - Went to my GP and he prescribed 2 creams; 1). Broad spectrum anti-fungal cream 2). Hydrocortisone cream (for inflammation). Then continue to use just the Broad spectrum anti fungal cream for another 2 weeks after. The creams were prescribed, so yes you may need to see your doctor. But honestly If you have never seen a doctor about an 'embarrassing' issue - honestly don't worry, It truly is their job and they will make you feel like it's just the same as putting a stethoscope to your chest - very routine. You need to go to the drugstore and buy Aveeno's bath packets containing collodial oatmeal. This stuff really works (I have excema). It will stop the itching pronto. You could buy liquid benadryl for itches which absorbs into the skin and numbs it but it might feel weird on your balls.
The reason your itchy there is because of the heat. You need to wash with an anti itch soap or glycerine soap. Dry off thoroughly. Then use talcum powder on the area. Or simple cornstarch. This will stop that.

This is for r11. When you have any kind of itch, heat will make it feel better. Heat causes the histamine, the source of the itch, to rise out of the skin. You can actually see it if you use a source of heat other than hot water, such as a hair-dryer. The heat causes the itch to become almost unbearable, and then these oily beads form on your skin and the itch disappears. The feeling is kind of like a yawn. It feels just too good. I think I'm talking about what you're talking about.
The problem is that it's a zero-sum game. The heat irritates your skin, causing the original itch problem to be replaced with a new itch problem.
I had poison ivy and thought the heat treatment was a godsend. It's almost orgasmic. So the rash that started on my thigh ended up covering both legs and most of my torso.
Heat is good if you can't sleep because of the itch but the next day, visit your doctor and get a round of prednisone.

Sounds just like what I go through. I scratch my sac with a hair comb for relief. I believe mine is caused by the medication I'm using. If you use any opiods they can often cause very itchy skin. There are times ive scratched so long and so hard that I open the skin and bleed. Funny thing is it only starts to itch when I start to scratch and then it explodes into a torrent. I'm not much help but your meds might be the cause??

I got this a few weeks ago. It started inside my navel and has moved down to around my dick. It is just an itch - no redness unless I scratch heavily and no flaking skin or any other obvious sign of fungus.
I tried Lanacaine to relieve the itching and that made it worse. A cooling gel relieved the itchy feeling but would not eradicate the problem. Clotrimazole gave the same results as Lanacaine. I have got myself a Lamasil spray and will see if that makes a difference. I really do not want to shave down there unless I have to. I am just assuming it is some form of jock itch although I don't have the tell-tale nappy rash symptoms.
Possible wheat intolerance. Time for a new diet.

the natural methods i preffer are
1.after bath gently wash your scrotal sack with hot water
2.during bath decrease the use of soap in scrotal area
3.after bath with a clean dry cotton cloath remove the water in scrotal area and with the help of hair drier or fan you just allow to warm up the scrotal area
4.reduce the use of inner wear during hot climates because the sweat will rise up the chance of fungus
5.clean your scrotum with your hands (with out using soap) to remove the sweatary waste
If it is non-specific dermititis then fungal creams and hydrocortisone will make things worse.

Well, although men can certainly get fungal infections, it 90% certainly isn't that, so don't waste too much time with any antifungals. If you want to eliminate a fungal, just take a duo (oral pill + clotrimazole cream, then you can strike that off the list. A sometimes itchy rash and other similar symptoms generally come under the umbrella of Red Scrotum Syndrome (RSS), but you already knew that. It's more common than what you'd think, and tends to happen to men in the second half of their life, but also earlier. Skin specialists don't really know of a remedy, there is little research (no money in it for drug companies), and basically no tried & tested known cure. Doctors and dermatologists will often give you a steroidal cream of some description, but these are well known for short term relief followed by a rebound and exacerbation of the problem (a lot worse). Use those creams at your own peril! It's actually interesting how many doctors will give you samples of creams from their top drawer for free (I wonder where they get those from?).
If there's an itch or a rash, as several other posters have said, it's caused by histamines, so something has triggered a histamine reaction. How to treat symptom? Antihistamines seem to work for many (block the H1 receptor). Don't use anything like Phenergen or Benadryl unless you want the nasty and sometimes long term side effects. Fexofenadine (many brands worldwide) are known to be effective in reducing that itch. Ask your doctor. They don't last long (short half-life) so research suggests more than 60mg twice daily, and many folk take 120mg twice daily (the 180 once daily does not last 24hours). Fexofenadine is one of the more safe antihistamines (google and read up on this) so you can generally try a dose recommended by your doctor, then adjust the dose. It's over the counter.
But this only relieves the symptom, it doesn't fix the underlying cause. Patch tests (on skin) are useful for eliminating triggers, they may not find the exact trigger for this condition. Often chronic idiopathic (no known cause) skin conditions are put down to psychological causes, stress, anxiety. You could draw links from these to almost every known condition, however there is no evidence or research to prove such claims. However, it's fair to say that stress and anxiety won't help you, so try to work out those issues in your life. Better to do that than take antipsychotic drugs, but try to relax either way.
The RSS may also be accompanied by folliculitis, and it's hard to know if shaving your pubes causes this or not (can do though), Some men are just prone to this condition. Doxycycline is an antibiotic that may help both the folliculitis and the RSS, and has seen some good results. Some other over the counter (OTC) remedies that will help and won't burn your nuts and bring tears to your eyes are zinc cream/nappy rash cream (the high zinc kind, eg Desitin), and baby oil seems to help many. Vaseline also seems to help, but it's hard to wear underwear with greasy zinc cream or Vaseline. Baby oil with aloe is almost tolerable, and you can put it on easily and cheaply 3 times a day without too much distress.
Sweat makes it worse, so needs to be avoided if possible. A roll on antiperspirant with high aluminium will help that, but don't use if you have broken skin (you may scream!).
Prevent rubbing of scrotum on inner thighs. If dry and not itchy, many men use talc powder to avoid the rubbing and soak up sweat. Avoid stuff like tea tree oil, or anything that is going to burn your sac and hurt like hell, it may numb the nerves for a bit but it won't solve the problem. Some of these things might help, but everyone is different. Hopefully someone may actually fund some research into the underlying cause of RSS and similar ailments. Cheers :-)
